Heart Disease is the number one killer of men and women in the United States. Preventing, treating
and caring for those with heart disease is our business: we are The California Society for Cardiac
Rehabilitation (CSCR). CSCR will be presenting their annual state conference March 4th-6th, 2010 at
Disney’s Paradise Pier Hotel in Anaheim. This letter is to request your support, which would help
provide a high caliber educational program to our attendees.
We are aware that your organization has been very supportive of various American Heart Association
and philanthropic community events. CSCR is a statewide, non-profit and nonpartisan organization
for health care providers involved directly with Cardiac Rehabilitation and Wellness Programs. We
are a state affiliate of the national organization, American Association of Cardiovascular and
Pulmonary Rehabilitation (AACVPR). The purpose of this non-profit public benefit corporation,
501(c) 3 is to advance education of cardiac risk factor management to both the medical community and
to the general public, and to support and encourage charitable and scientific programs concerning
cardiovascular rehabilitation by:
1. Providing education for health care professionals working in cardiac rehabilitation.
2. Encouraging and promoting public awareness of the nature of cardiac rehabilitation
services.
3. Promoting scientific research in cardiac rehabilitation.
4. Providing communication and networking opportunities to members on an ongoing basis.
5. Advancing the standard of care in cardiac rehabilitation through standards in quality
assessment and improvement.
6. Obtaining and disseminating information regarding reimbursement to cardiac rehabilitation
services.
Our organization’s membership is 183 and includes 140 Cardiac and/or CardioPulmonary
Rehabilitation programs in the state of California and two out of state programs (Texas and Oregon).
Thirty-eight of these programs have been nationally certified by the American Association of
Cardiovascular and Pulmonary Rehabilitation (AACVPR). This year’s conference planning committee
consists of cardiac rehabilitation specialists from several southern California medical centers:

Clinical Nurse Specialists, Nurse Practitioners, Educators, Physical Therapists, Registered Dietitians,
Pulmonary Rehabilitation professions, Physicians, Students, Certified Diabetes Educators, and Health
Care Administrators who have direct patient contact and decision-making ability for purchase in the

Our theme, “From Pier to Peer – the 2010 CSCR Educational Conference” emphasizes practical
applications of scientific research and practice surrounding cardiovascular disease. Our goal is to
increase clinical and practical knowledge, sharpen assessment skills, broaden our referral base,
enhance clinical practice and improve patient education and self-management. We have included
