APPENDIX “A” BA 790-800-200 January 2000 VERIZON REAL ESTATE (VZRE) CONTRACTOR REQUIREMENTS AND METHOD OF PROCEDURE (MOP) DEFINITION: This document sets forth-mandatory requirements for all Contractors that perform building construction related work on Verizon (VZ) premises. A primary purpose of this document is to eliminate telephone service interruptions that are caused by construction and / or repair work activities. ADMINISTRATION: A copy of this document shall be included as a part of the contract documents for every building Construction and / or repair project. The Contractor is responsible for the adherence of its subcontractor(s) to this document and shall sign, obtain necessary signatures and provide a copy to VZ prior to the start of work. A violation of the policies set forth herein may be a material breach of the contract between VZ and Contractor and may result in removal of Contractor from VZ’s qualified bidders list. Contractor agrees to reimburse VZ for the costs of prosecuting any claim (including legal fees) resulting from any violation of the policies set forth herein by contractor that result in a telephone service interruption or financial loss to VZ. REFERENCE DOCUMENT: In this document reference is made to the Verizon Network Equipment Installation Standards document BA - IP722001. This standard can be found on the VZ Corporate Web at http://baimsa.bellatl.com/bap/ied/ip/pdf/i72201.pdf 1.0 REQUIREMENTS FOR WORKING IN ALL Verizon FACILITIES: Contractor or its subcontractors must: 1.1.1 General Topics 1.1.2 Notify the appropriate VZ Real Estate (RE) Representative1 at least forty-eight (48) hours prior 1.1.3 1.1.4 1.1.5 1.1.6 to starting a project, via voice mail, Email and or fax. Dirt and debris from cutting drilling demolition, etc. shall be removed at the end of each work activity or before leaving the job site each day to prevent tracking dirt and debris through the building. A damp mop, tack cloth or HEPA vacuum shall be utilized. Contractor shall not use VZ’s trash bins or dumpsters. All such dirt and debris shall be removed from work site daily. Contractor shall not assume that VZ will provide trash bins dumpsters or recycle containers for contractor’s use. All such dirt, debris and recycle material shall be removed daily. Refer to paragraph 1.5.3 for disposal compliance information. Contractor parking, job staging and tool / equipment storage shall be as directed by the appropriate VZ RE representative. Contractor shall not assume that VZ will provide parking facilities for contractor’s employees or vehicles. Contractor is not to assume VZ will have an “on site” material storage area. Official VZ phone service is for VZ use only. Contractors shall provide their own phone service per contract requirements. In special cases Contractors’ use of official VZ telephones shall be allowed only when authorized by the VZ Manager who is responsible for the official service. Smoking is not allowed in any VZ facility. This includes roof areas and exterior areas within 20 feet of an entrance. 1.2 Risk Analysis 1.2.1 Complete a risk analysis with the VZ RE Representative prior to the start of work. The risk analysis shall specify the designated routes through VZ’s premises for Contractor’s work activities and the potential for service interruptions due to all planned work activities. It shall also include any potential interruptions due to poor indoor 1 Property Management Team Leader, Construction Services Team Leader, Construction Professional, Project Manager, Construction Manager 1.2.2 1.2.3 1.2.4 1.3 air quality (IAQ), electrical power failure, HVAC failure, fire, false fire alarms, excessive noise, water, dust, dirt, etc. In accordance with the risk analysis and requirements that may be further described in the contract documents, the Contractor shall take all precautions necessary to prevent service interruptions including preparing and following detailed Methods of Procedure (dMOP) approved by the VZ RE representative and local Central Office (CO) supervisor. These Mop’s shall describe the work being performed, the manner to implement the work, what circuits of equipment will be effected, the length of any planned outages, any available battery/ power plant reserve requirements, timing of the work, what temporary facilities will be provided to minimize the impact on the existing building, who will be responsible for and involved in performing the work, back out procedures and all other pertinent details (see paragraph 2.2. Safe Time Practice). The MOP shall identify at what points the dMOP can work be stopped if necessary. The dMOP shall then identify what would be done at the point to restore the system(s) and how and when that restoration work would be done. The dMOP shall also identify the “point of no return”, that is, the point in the job where we can no longer turn back to restore, but must continue forward and complete the new work in order to have our systems functional. Prior to starting any work that may pose a potential risk of activating the fire alarm (i.e., due to any open flame, smoke or dust generation, excessive vibration, soldering, brazing, welding, painting, etc.), call the Network Control Center (NCC) for CO work only on ________________ and the Real Estate Customer Support Center (RECSC) in the Mid-Atlantic2 area on 800-685-1608, or the RECSC for the New York / New England3 area on 888-696-3973 or 800-339-6144 in New York (try 1-888 # first) for operational assistance. Also notify the appropriate VZ RE representative. If the fire alarm is accidentally tripped, notify both the NCC and the RECSC in the New England / New York area or the Mid-Atlantic area, at the numbers above to avoid fire department dispatch. See paragraph 1.4.1. Prior to any floor and or wall penetrations contractor shall probe for imbedded services. Method of Procedure (MOP) 1.3.1 Contractor shall prevent and shall be responsible for the costs, which arise from all service 1.3.3 1.3.3 1.4 1.4.1 1.4.2 1.4.3 1.4.4 1.4.5 interruptions due to Contractor’s work operations, irrespective of whether or not the potential interruption was identified during the risk analysis. Attachment “B “ of this document provides a copy of the MOP to be used for the development of all "General" and "Detailed" MOP’s for VZ RE. The dMOP shall identify what points in the MOP can work be stopped if necessary. The dMOP shall then identify what would be done to restore the system(s) as well as how and when the restoration work can be done. The dMOP shall also identify the “point of no return” that is, the point in the job where the work can no longer be turned back to restore the system(s), but must continue forward with the work to have the system functional. All contract work shall be performed in accordance with the days and times specified in the MOP, item # 5. Security Maintain building security at all times. All exterior entrance doors must be kept locked, fire doors must be kept closed and all temporary openings through exterior walls must be secured when leaving facility. Exterior doors shall be monitored while opened for the passage of construction materials. Contractor must obtain either a VZ non-employee contractor ID card or a VZ visitor pass for all employees to prominently display at all times while on VZ premises. VZ may, at its discretion, request a background investigation of any contractor’s employees, agents, or the employees of subcontractors or vendors who work on VZ premises. Also, VZ may deny or delay access to any VZ facility for any of contractor’s employees agents, or the employees of the sub-contractors or vendors. Keys / key cards shall not be transferred to other users. Access cards will expire upon completion of the contract or at the expiration of the holders VZ Contractor ID card. Local Security practices must be followed on VA projects. Check with the VZ representative and / or Building Coordinator. 1.5 Safety 1.5.1 Maintain a copy of Material Safety Data Sheets (MSDS) on job site for all hazardous materials used and submit a copy(s) to the appropriate VZ RE representative for project file and the Environmental Group (800-386-9639). 2 New Jersey, Pennsylvania, Delaware, Maryland, District of Columbia, Virginia, West Virginia 3 Maine, Vermont, New Hampshire, Massachusetts, Rode Island 1.5.2 1.5.3 1.5.4 1.5.5 1.5.6 1.5.7 1.5.8 1.5.9 1.5.10 1.5.11 1.5.12 1.5.13 1.6 1.6.1 1.6.2 1.7 1.7.1 1.7.2 1.7.3 Contractor shall be on the alert for potential asbestos containing material (ACM). If ACM is discovered, Contractor shall stop work and notify the appropriate VZ RE representative(s) or VZ Project Manager – Asbestos services before proceeding. (Refer to paragraph 1.2.3). Disposal of hazardous waste and recyclable material (corrugated boxes, office paper, glass, plastic bottles, aluminum cans, etc.) must be arranged through the Environmental Group, which can be contacted at 800-3869639 and selecting option 2. Disposal of PCB ballast’s, non-PCB ballast and fluorescent lamps must be coordinated through the Environmental Group. PCB ballast’s shall be drummed and sealed for shipment directed by the Environmental Group. Fluorescent lamps shall be package per the Environmental Group instructions for shipment as directed by the Environmental Group. The Environmental Group must receive paper work in compliance with all applicable federal, state and local regulations and VZ standards. If a spill occurs (fuel, oil, acid, etc.) or suspect ACM has been disturbed take immediate action to Contain spill and then contact the Environmental Group on 800-386-9639 and select option 2. VZ Infopoints documents regarding hazardous and recyclable materials are available from the VZ RE representative. In general, contact the hotline to report any emergency, such as injury or occupational exposure and select the applicable option from the menu list. Also notify the VZ RE representative for the project. (See paragraph 1.2.3). The indoor use of propane or gas powered mechanical tools is strictly prohibited. The contractor must understand and shall adhere to all local, state and federal safety regulations. It is the Contractor’s responsibility to insure the employees are properly trained to minimize safety hazards and how to safely operate the equipment that they use to perform the required work tasks. Contractor shall provide and require employees to have personal protective equipment (PPE) necessary to safely perform the work task. Contractor’s employees shall receive adequate training for safe entry and work habits in confined spaces during boiler work. All permits require for confined space entry shall be posted. Contractor shall insure that its employees understand and follow lockout / tag out procedures while working on hazardous energy sources. Contractor shall provide appropriate fall protection while working on the roof or otherwise aloft. Power, pneumatic and explosive charged actuated fasteners could be used only with prior approval and specific authorization from a VZ representative. Fire Safety Storage of flammable or combustible materials in any VZ facility is prohibited unless approved by the appropriate VZ RE representative. No type of portable heating equipment is allowed in a VZ facility. All cable hole / floor penetration shall be fire stopped to the required performance level established by VZ at the end of each work activity or before leaving the job site each day. Indoor Air Quality (IAQ) Any work consisting of odors from material such as paint, sealers, chalking, adhesives, cleaners, etc. shall have an MSDS sheet as per paragraph 1.9.1 below. All work associated with the above type material requires a negative air machine vented outside of the building envelope. This work will be done after normal working hours in the facility and the negative air machine will be operational during the work procedure as well as a sufficient period of time to insure the work area odors have dissipated. The work area must be sealed off from all other areas in the facility to insure the odors from the work activity will not migrate to others areas of the building. 1.8 Logs / Emergency Information 1.8.1 An “Early Warning Fire Detection Log” shall be kept at a designated job site area, preferably at the contractors job phone. This log is to be filled out at the beginning of the work tour and the end of the work tour when a work procedure has the potential to trip the fire detection system. These work activities may include but are not limited to: any type of construction or repair work, system tripping by work on the fire detection system or any possible accidental tripping of the fire alarm system due to other con-striation activity. Contractor is to call the NCC, for CO work only, and the RECSC in the Mid-Atlantic4 area on 800-685-1608, or the RECSC for the New York / New 4 New Jersey, Pennsylvania, Delaware, Maryland, District of Columbia, Virginia, West Virginia England5 area on 888- 696-3973 or 800-339-6144 in New York (try 1-888 # first). See paragraph 1.2.3. See attachment “D “. Complete an “Emergency Information” sheet as part of the required emergency response plan and post it at the work site during the entire contract work period. Provide a copy to the appropriate VZ RE representative for distribution to the appropriate parties within VZ. See attachment “A”. Immediately report telephone equipment damage and / or service interruptions to VZ in accordance with the list on the “Emergency Information” sheet and in accordance with the MOP. 1.8.2 1.8.3 1.11 Permits 1.9.1 No welding, cutting, brazing, soldering or open flame from oxy-fuel gas or propane powered torches will be allowed in or around any VZ facility without a VZ approved “Welding Permit”. Prior to approving any welding, cuttings, brazing or soldering in a VZ facility the VZ RE representative must inspect the work area and confirm all precautions have been taken to prevent a fire. See attachment “C”. 1.10 Excavation 1.10.1 Prior to any excavation the contractor must request an underground utility mark-out showing all underground utilities that are on site. Hand digging must be done to identify location of all underground utilities within 18” of underground utility mark-outs. In addition, contractor must use “One Call” procedures during excavation. 1.11 Supervision 1.11.1 The Primary Contractor must provide dedicated, English speaking, on-site supervision. The on-site supervisor must be trained in safety practices applicable to the work tasks being performed. The on-site supervisor shall be on-site at all times including when a subcontractor is the only one performing work. Supervision changes are prohibited without prior VZ approval. Contractor’s on-site supervisor shall have the authority to make all necessary decisions for the contractor to ensure compliance with the requirements set forth in this document and with all project specifications and drawings. 1.11.2 VZ may, at its discretion, employ the services of an Owner’s Representative. This decision will not relieve the contractor from fulfilling its requirements under paragraph 1.1.20 for providing a dedicated, on-site supervision at all times. 1.11.3 Any VZ employee or Owner’s Representative is authorized to order the immediate stopping of work that endangers employees, equipment or services. The appropriate VZ RE representative must be notified immediately of this action (Refer to paragraph 1.4.2). 2.0 2.1 REQUIREMENTS FOR WORKING IN CENTRAL OFFICES (CO's)OR FACILITIES WHICH HOUSE TELEPHONE EQUIPMENT (SWITCHES, TOLL/TERMINAL EQUIPMENT AND BATTERY / POWER ROOMS) Construction Precautions / Requirements 2.1.1 No work shall commence directly above or adjacent to equipment frames or power equipment without the provision of physical protection as specified in the contract documents or as required by the on-site risk analysis (see 1.2 Risk Analysis), and VZ shall approve such protection or analysis (both the VZ RE representative and the CO Supervisor). Contractor’s personnel shall not walk through switch aisles that have not been physically protected. A Network Awareness Binder is available from VZ for training of contractor’s employees. 2.1.2 When required by the contract documents, telephone equipment is to be completely isolated from dust and dirt by temporary partitions constructed of fire resistant materials (plastic sheeting shall also be reinforced & anti-static) with securely taped joints. Negative air pressure shall be maintained in the enclosed construction area all times with an air system exhausted to the building exterior. Before any demolition work starts the VZ Team Leader and CO Supervisor must approve the partitions. Prior to partition removal, the protected area and partitions must be cleaned and the VZ Project Manager and CO Supervisor must approve such cleaning. 2.1.3 If the potential exists to short out any exposed DC bus or battery cell links, the contractor must encapsulate the exposed DC bus with rubber blankets and plastic tie-wraps. The Contractor must also provide appropriate insulating coverage for exposed battery cell links. No other work will start within the building, including the movement of materials into the building until this work is completed and approved by the VZ Team Leader and CO Supervisor. 5 Maine, Vermont, New Hampshire, Massachusetts, Rode Island 2.2 Safe Time Practice 2.2.1 In accordance with the VZ Regional Operations Safe Time Practice (current issue of BA 002-300-100), all work which has the potential to severely impact customer service will be performed during a time period when a disruption of service would have the least effect on customers. This time period for, general safe time, is between 11:00 PM until 7:00 AM,. However, safetime for work on power and / or electrical systems is 12:00 AM until 6:00AM, or as approved by: (1) the Second Tier support (for a local contact, call the Network Service Assurance Center, (“NSAC”) at ___________________) and (2) by local management. NO WORK may take place during the day in a working or power equipment aisle, or within 10 feet of this type of equipment, without the concurrence of local operations management personnel. Any work above or within 10 feet of DC power equipment (rectifiers, buss bars & batteries) shall be done during the Safe Time Period. If it is necessary to work outside of this period, the Second Tier of support (see NSAC contact information above) should be included in the decision making process. Detailed MOPs are required when working on AC panels, above telephone equipment, in power rooms, etc., and must be approved by the Central Office supervisor before any Safe Time work is started. 2.3 Not Permitted in Central Offices 2.3.1 Absolutely no use of wireless transceivers (walkie-talkies, cellular phones, PCS or cordless telephones/headsets) or cameras with flash attachments is permitted in any Central Office. These devices must be powered off when in these spaces: electrically powered motor driven tools, HEPA - vacs and negative air fans when used within 25 feet of telecommunications systems. This equipment shall only be used between the hours of 12:00a.m. through 6:00 a.m. and with prior VZ permission and as well as appropriate precautions and protection taken. Water shall not be utilized at any floor level above a switch without the appropriate precautions and protection taken. The use of electrical receptacles mounted at the base of telephone equipment frames, frame end guards, and main distributing frames is strictly prohibited. The use of brooms is prohibited.
