Summary
CMHA HKPR Branch is an accredited Mental Heath agency seeking a part-time Registered Nurse or Registered Practical Nurse
to provide assessment and facilitation for members of the community accessing the Ontario Telemedicine Network (OTN) as a
means of obtaining consultation and assessment from specialists across the province of Ontario through OTN videoconference
and in person, as arranged.
Responsibilities









Receives and facilitates OTN referrals through email, faxes and telephone inquiries and walk-in contacts;
Contacts physicians’ offices to obtain patient information;
Communicates with referred individuals and doctors to schedule clinic appointments via OTN scheduling network,
and on site for in-person consultations;
Assists and supports patients through the process of medical or psychiatric consultation. This includes, but is not
limited to:
o Attending OTN consultations with patients and doctors, when required, to conduct physical assessments or
other nursing duties, as required, by the consulting physician during the consultation;
o Assisting patient and doctor with communication, as well as understanding instructions from physicians;
o After consultations, supporting patients as needed in dealing with the immediate impact of the consultation
and clarifying the outcomes and follow-up instructions for patients;
o Assisting patients with other referrals as ordered by consulting physician or as requested by patients;
Uses email and telephone to send follow-up appointment reminders and related communications to patients and
their support staff;
Performs various general administrative tasks including emailing, faxing and photocopying patient information;
Maintains patient medical records in a confidential and organized manner and completes related electronic and hard
copy documentation in accordance with CMHA OTN policies and procedures;
Files and/or scans reports and faxes on CRMS;
Transcribes and follow doctor’s orders including possible administration of medications.
Qualifications


The following are requirements of this position:
o Being a Registered Nurse or Practical Nurse in good standing with the Ontario College of Nurses, RNAO or
RPNAO;
o Four or more years of related experience is required, with experience in a mental health field being preferred;
o Demonstrated ability to take initiative and develop new programs to meet identified gaps in service;
o Experience with administration, injection, side effects and iatrogenic manifestations or psychotropic
medications, especially the newer neuroleptics;
o Solid health assessment skills, sound clinical judgment and decision-making skills;
o Ability to provide education to staff, patients and community members about health and wellness, and
medication management;
o Excellent communication and interpersonal skills;
o Ability to work collaboratively with other members of the community;
o Strong organization skills;
o Solid computer skills; knowledge of Outlook, Excel, PowerPoint and electronic patient records;
o Satisfactory criminal reference check and vulnerable sector screening;
The following are assets of this position:
o Knowledge of Information Technology Systems (e.g. OTN equipment knowledge);
o Knowledge regarding Metabolic Syndrome and management;
