2. JOB PURPOSE

APTS (0.5wte)

To provide Clinical Psychology Services to patients referred by Healthcare Practitioners within extended

Primary Care Teams and local Community Mental Health Teams (CMHT).

To formulate and deliver responsive, specialist psychological assessment and treatment at individual and systemic levels.

North Perthshire CMHT (0.5 wte)

To have responsibility for managing the day-to-day provision of expert, comprehensive, specialist psychological services to adults experiencing severe and enduring mental health problems and to colleagues and organisations concerned with the delivery of health and social care to the patient group in the North Perthshire area as part of the North Perthshire Community Mental Health team

APTS and North Perthshire CMHT

To offer advice and consultation on psychological care to non-psychologist colleagues, working autonomously within professional guidelines.

To supervise graduate, trainee and accredited psychological therapy staff employed in the specialist multidisciplinary team, and to manage aspects of their work as directed by the Lead Clinician.

To develop, facilitate and evaluate service and policy developments within the area as well as within the specialty as directed by the Lead Clinician.

3. ORGANISATIONAL POSITION

4. SCOPE AND RANGE

The APTS Service accepts referrals on behalf of patients registered with one of the 28 GP practices located within Perth and Kinross. Perth and Kinross Adult mental health speciality is based in Murray

Royal Hospital, but the clinical psychologists working in the community mental health teams are located within their respective team bases. The service for primary care (APTS) provides a multi disciplinary adult psychological therapies service. The total establishment for Adult Mental Health

Psychology in Perth and Kinross is 1.9wte clinical psychologists (CMHTs), 0.5 wte clinical psychologist for community rehab and recovery (Perth and Kinross), and within APTS 4.0wte clinical psychologists (including the lead clinician); 4.1wte clinical associates in applied psychology and

0.8wte CBT The role of the Clinical Psychologist involves direct patient contact and contact with significant others (e.g. relatives, carers, other professionals) for the purpose of psychological assessment and treatment, the co-ordination of referrals and waiting lists for designated GP practices as delegated by the Lead Clinician, teaching and supervision of Trainees and Assistant

Psychologists and to participate in regular audit, service development and service evaluation activity.

NHS Tayside Clinical Psychology service to community mental health teams is part of the adult psychology service for Perth and Kinross. There are three Community mental health teams across

Perth and Kinross; Perth City, South Perthshire and North Perthshire CMHTs.

The community mental health teams are multi-disciplinary and deliver mental healthcare to those people with severe and enduring mental illness such as schizophrenia, bipolar disorder and personality disorder.

Consequently, the postholder will;

For APTS role

 Be responsible for highly specialist psychological provision for general practice within Perth and

Kinross, to assess needs within Primary Care and formulate response (e.g. input to case allocation, contribute to policy)

 Manage autonomously within professional guidelines a caseload of AMH/Primary Care referrals from the locality, which require highly specialist, clinical psychological assessment and intervention

For CMHT role

be responsible for specialist psychological provision for the locality CMHT, to assess CMHT needs and formulate and deliver an appropriate response (e.g. input to case allocation, contribute to policy)

have sole responsibility for representing Clinical Psychology at CMHT weekly clinical meetings and case review meetings and ward meetings as necessary

have sole responsibility for co-ordinating the administration of referrals, communication with clients and with referral agents and other parties at the time of referral and throughout the assessment & treatment process for both roles:

 Manage and supervise the caseload of assistant or trainee clinical psychologists as directed by Lead

Clinician

 Provide clinical supervision to assistant psychologists, trainee clinical psychologists, cognitivebehavioural psychotherapists, post-registration clinical psychologists, and staff from other disciplines within the CMHT as required

 Provide expert consultancy and advice to colleagues, Primary care team including general practitioners, NHS organisations and other bodies such as Social Services, voluntary and nonstatutory agencies as required.

 Ensure that services are deployed in line with best contemporary standards and ethical frameworks, principles of equity and reflecting evidence based practice

 Undertake clinical audit to ensure maintenance of professional standards and research as appropriate, with particular regard to Clinical Governance.

 Provide specialist teaching and training to professional trainees, colleagues and organisations as required.

5. MAIN DUTIES/RESPONSIBILITIES

Clinical

 To provide highly specialist psychological assessments of patients referred to the service, based upon the appropriate use, interpretation and integration of highly complex data from a variety of sources including: psychological and neuropsychological tests, self-report measures, rating scales, direct and indirect observations, and semi-structured and structured interview schedules with patients, family members and others involved in the patient’s care.

 To provide diagnosis and treatment recommendations for highly complex clinical disorders which may require onward referral to other specialist services requiring NHS Tayside funding

 To formulate and implement plans for the formal psychological treatment and/or management of a patient’s mental health problems, based upon an appropriate conceptual framework of these problems, and employing methods based upon evidence of efficacy, across the full range of care settings.

 To be responsible for implementing a range of psychological interventions for individuals, carers, families and groups, adjusting and refining psychological formulations drawing upon different explanatory models, and maintaining a number of provisional hypotheses.

 To evaluate and make decisions about treatment options, taking into account both theoretical and therapeutic models and highly complex factors concerning historical and developmental processes that have shaped the individual, family or group.

 To exercise autonomous professional responsibility for the assessment, treatment and discharge of patients.

 To provide highly specialist psychological advice, guidance and consultation to other professionals contributing directly to patients, formulation, diagnosis and treatment plans.

 To contribute directly and indirectly to a psychologically based framework of understanding and care for the benefit of all patients of the service, across all settings and agencies serving the care group.

 To undertake risk assessment and risk management for individual patients, and provide advice to other professions on psychological aspects of risk assessment and risk management.

 To communicate, in a skilled and sensitive manner, information concerning the assessment, formulation and treatment plans of patients under their care and to monitor progress during the course both uni- and multi-disciplinary care. Within the North Perthshire CMHT role, it may be necessary to respond to and communicate information in respect of patients requiring urgent and emergency assessment as part of the multi-disciplinary team, which involve on site, domiciliary, or inpatient assessment

 To provide expert advisory and consultancy services to colleagues and organisations within Perth and

Kinross and elsewhere in the wider healthcare systems as appropriate

 To supervise staff from other disciplines (e.g. Counselling, Clinical Specialist in Cognitive Behavioural

Therapy) who are employed in the locality as well as Clinical Psychologists in Training and Assistant

Psychologists. Provide clinical supervision to staff from other professions who are involved in delivery of psychological therapies within the North Perthshire CMHT.

 To evaluate the performance of Trainee Clinical Psychologists, where necessary, as directed by the appropriate clinical training course.

 Provision of impromptu supervision, support and advice (e.g. on risk assessment) for colleagues and junior staff

 To ensure the receipt of regular clinical professional supervision from a senior clinical psychologist and, where appropriate, other senior professional colleagues.

Management and Recruitment

 To manage the workload of assistant psychologists and trainee clinical psychologists within the framework of the Department’s policies and procedures, as delegated by the Lead Clinician.

 To be involved, as appropriate, in the short-listing and interviewing of Assistant/Graduate

Psychologists.

 To ensure that waiting lists are managed in a professional manner with due attention to patients and to referring agents communications and correspondence, as delegated by the Lead Clinician

 To provide representation of clinical psychology on committees and working groups as directed by the

Lead Clinician, or by the Head of Service. Also to represent the Lead Clinician as required

Research, Policy and Service Evaluation

 To ensure the policies of Department are carried out efficiently and to the best possible standard.

 To keep management well informed about the needs and developments of the service and all psychological matters concerning adult mental health in primary care.

 Monitor monthly statistics relating to the specialty for accuracy, and to audit trends (e.g. growing waiting times) that indicate a need for service developments (e.g. waiting list initiatives).

 To review and utilize theory, evidence-based literature and research to support evidence-based practice in individual work and work with other team members.

 To conduct and advise on appropriate clinical research and audit, organizing and supervising trainee research projects and thesis, as required

 To contribute to service related research which is relevant to the department, delivery and evaluation of psychological interventions.

 To manage research budgets, as required, when acting as a lead researcher on specific funded projects.

6. COMMUNICATIONS AND RELATIONSHIPS

 To communicate clearly, professionally and empathetically with patients, their relatives and carers, in a wide range of sometimes very distressing and emotive circumstances, which may involve imparting sensitive clinical information relating to the development and prognosis of an individual's psychopathology.

 To communicate promptly, clearly and sensitively with referral agents and colleagues providing a written history of the presenting complaint, the clinical findings on examination, a clinical formulation, diagnosis, and a recommended course of action; ensuring that these observations are sent to all those with a significant role in the management of the patient, and that an appropriate record is stored in the records system.

 Be experienced and skilled in developing a therapeutic alliance with patients whilst maintaining a professional and ethical standing

 To ensure a presence or written report at important decision-making meetings concerning patients to ensure the passage of relevant information to colleagues and participation in the process (e.g.

The terms and conditions of service for this post are those determined by the NHS Staff Council.

The current salary scale for the post is £38,851 to

£46,621 per annum (pro rata for part-time staff). Placing on the scale on appointment is normally at the minimum but may be higher subject to verification of previous relevant service.

Salary is paid Monthly by Bank Credit Transfer.

The hours of the post are 37.50 per week.

Start and finish times will be determined by the needs of the service.

Membership of the NHS Superannuation Scheme is not compulsory but is open to all staff between the ages of

16 and 70 (65 in some instances).

The contributions paid are a percentage of superannuable pay, which is essentially basic pay excluding, for example, overtime or travelling expenses.
