This form is to record the use of an injectable anesthetic (e.g., ketamine/xylazine, pentobarbital), used for a non-surgical procedure, from which the animals recover. It is not necessary to fill out a section for each individual animal. Serial procedures can be noted on one section (e.g., started anesthesia on 5 mice at 8:00 am, all up and recovered by 10:00 am). Documentation is not required when a gas anesthetic is used (including CO2/O2) that takes less than 1 minute from which to recover. However, animals must be continuously observed until they are conscious from the gas anesthesia.
