This form must be typed. Handwritten forms will not be accepted.

The completed form, signed by the student and the primary supervisor, should be submitted to the appropriate Faculty
Postgraduate Office when the thesis/exegesis is lodged for examination. If the application is approved by the Faculty
Postgraduate Committee, the form will be signed by the Dean and sent to the University Postgraduate Centre for insertion into
