We are committed to providing our patients a high standard of care in a friendly

patients. This leaflet is designed to provide you with essential information
regarding the surgery and the services available to you as our patient. If you have
any suggestions as to how we can improve our services, please contact the Practice

Walk-in Centre
For minor injuries you can attend Teddington Memorial Minor Injuries
Unit, which is in Hampton Road, Teddington. This is open from 7am to
10pm Monday to Friday and 8am to 9pm weekends and bank holidays.

In an Emergency
Whatever the day or time if you or someone else experiences severe chest
pain, loss of blood or suspected broken bones go to the nearest accident and
emergency department, or call 999.
Accident and Emergency departments are open 24 hours a day, 365 days a year and
can assess serious injuries and provide emergency treatment. Our nearest Accident
and Emergency Department is located at West Middlesex University Hospital,
Twickenham Road, Isleworth, Middlesex, TW7 6AF.
THE PRACTICE NURSE carries out a wide range of nursing procedures
at the practice. She is also trained to conduct a wide variety of well-person
screening including running the practice’s cervical smear and immunisation
clinics. She can give health promotional advice, monitor some long-term medical
conditions and provide some contraceptive advice. She can offer dietary advice,
travel advice, administer travel vaccinations, influenza vaccinations and routine
and childhood immunisations. She can also advise on giving up smoking and
alcohol. She will in conjunction with the doctors, advice on asthma, diabetes,
blood pressure, cholesterol and heart disease. She is supported by the health care
assistants who are trained to take blood samples, blood pressure monitoring and
general health checks.
THE HEALTH VISTORS
are general nurses with further training in
providing health care for all ages, but especially for families with young children.
They offer a service at home or at the Health Clinic. They can be contacted at the
Clinic between 9:00am and 4:00pm on 020 8321 2258.
THE DISTRICT NURSE
works with the doctor and performs nursing
procedures in your home for anyone unable to attend the surgery.
THE PRACTICE MANAGER is responsible for the day to day running
of the practice. She is available most weekdays and if you have any problems
using our health service, she will be only too pleased to assist you.
THE PRACTICE RECEPTIONISTS
A number of appointment slots are available at the beginning and end of the day for
commuters. These can be booked up to 4 weeks in advance. You must be prepared to
see any doctor at the practice. You can expect to see a doctor within 2 working days.
If however you express a preference for a particular doctor you may have to wait
longer. Please be on time for your appointment.
We aim to see patients within 20 minutes of their appointment time. If not, an
explanation will be given. If for any reason you have been waiting for over 30
minutes please speak to the receptionist on duty.
TELEPHONING THE SURGERY
We can be contacted on 020 8577 9035 24 hours a day 7 days a week. The clinicians
are available to take telephone calls at set times during the day. If you need to speak
to one of them, please ask at reception when they are next available. You can make a
telephone consultation appointment.
HOW TO MAKE AN APPOINTMENT FOR A BLOOD TEST
You will need a form from the doctor or nurse to book an appointment. You will be
required to fast for 12 hours for glucose or cholesterol monitoring
The phlebotomist is available every morning, by appointment only, for blood tests
arranged by the doctor or nurse. Appointments can be made in person or by
telephoning the surgery.
HOW TO OBTAIN THE RESULTS OF YOUR BLOOD TESTS
OR OTHER INVESTIGATIONS

telephoning for medical attention, the receptionists may ask you for a few details.
They have been trained to make these enquiries so that we can help you in the most
appropriate way. They are trained to answer general but not medical queries.
Please telephone the surgery for your results. It is in your interest to do so because
there will always be a message for you. Do not assume that if you have not heard
anything that no action is required. We will contact you by telephone or letter for
significant results that require immediate action such as new medication. However
we process many results that are borderline or need to be repeated in 1 year and we
do not have the resource to contact everyone in such circumstances.
If you have any queries about test results, hospital appointments, ambulance
bookings or anything else that you are not too sure whether to bother the doctor,
please speak to one of team.
Please do not call for results in the morning as reception is very busy at this time.
We operate a full appointment system. Reception is open from 8:00am every
weekday morning to make appointments. Please telephone 020 8577 9035. You
will always be seen on the same day in case of an emergency.
KEEP IT OR CANCEL IT! DON’T BE A DNA (DID NOT
ATTEND)
If you have made an appointment to see any member of the health care team and no
longer require it please call us and cancel it. Someone else will be grateful for that
appointment.
HOME VISITS
DISABLED ACCESS
Please try and get to the surgery as we can offer a more comprehensive service
here. However if you require a visit, requests for home visits should be made by
10:00am whenever possible and should be limited to when you are bed-bound by
sickness. The Doctor will discuss the problem with you before deciding whether a
home visit is necessary. Requests for visits are assessed by the doctor on medical
grounds. Lack of transport is NOT in itself a reason for a visit. We would expect
patients to arrange transport and attend the surgery in most circumstances.
We have 3 parking spaces reserved for patients displaying a disabled sticker.
Access for patients in wheelchairs is by ramp to the front door. We have toilet
facilities which are suitable for use by patients in wheelchairs. Please ask for help
if you require it.
REPEAT PRESCRIPTIONS
If you are on long-term medication repeat prescriptions can be obtained
submitting an authorised written request preferably using the repeat prescription
tear off slip or a letter stating the name of the medication and dosage (not ‘my
white/pink/blue tablets/inhaler’ etc). These can be collected 48 hours later (not
including weekends). Please therefore ensure that you have enough medication to
last you until you receive your next prescription. Alternatively, if you wish,
prescriptions can be requested by post, but please remember to enclose a stamped
addressed envelope when you write to the surgery. Repeat prescriptions
CANNOT be requested over the telephone as errors can easily be made.
Repeat dispensing is available for patients whose medication is long-term and
unchanging. Please ask the doctor or receptionist for details.
INFORMATION ON SERVICES
SCREENING SERVICES
Many diseases are preventable. We offer health checks to discuss with you any
risk factors to lessen the chances of you having a heart attack or a stroke. Most
will be with practice nurse, but early morning appointments with the doctor are
available to help commuters.
We offer and encourage our patients to have the following health checks
NEW PATIENTS – as soon as you register with the practice
PATIENTS OVER THE AGE OF 75 – once a year unless you are being seen
regularly at the practice
ALL PATIENTS – every 3 years if you have not been seen for another reason.
WOMEN’S HEALTH
ANTE-NATAL
If you are considering pregnancy in the future you must ensure that there is no
danger of you contracting German Measles (Rubella) during the pregnancy. Ask
the nurse for a blood test even if you think you were immunised as a child. If
ZERO TOLERANCE
All staff at the surgery have the right to conduct their duties without the threat of
violence. The surgery has a policy of removing from the list any patient who is
physically or verbally abusive or threatening towards any member of our staff or
other patients. We undertake to treat all our patients with respect – in return we
feel that our team is entitled to the same respect.
FEEDBACK AND COMPLAINTS
LET US KNOW
We welcome feedback as a positive way of improving our services. Suggestion
cards are available at reception. The practice undertakes a comprehensive annual
survey and involves patients in discussing the results.
We do our best to provide a friendly efficient service, however should you
encounter any problems please inform the Practice Manager/receptionists. We
will do our best to improve/resolve any difficulties that you may be experiencing.
The Practice Manager is responsible for our in-house practice complaints
procedure. An information leaflet is available at reception.
THE TEAM
THE DOCTOR - Dr Sunil Mayor qualified in Nairobi, Kenya in 1979 and
has been practising as a general medical practitioner since 1988. He speaks
English, Hindi, Punjabi, Gujarati and Swahili. He offers a full range of general
medical care.
Staff involved in your care will give you their names and ensure that you know
how to contact them. It is very important that you notify us of any change of name
and address. Please note that if you move out of our practice area, it will be
necessary for you to register with another GP locally. A map of our practice area
is displayed at reception.
It is our job to give you treatment and advice. In the interests of your health it is
important for you to understand all the information. Please ask us if you are
unsure of anything. We aim to work with you in partnership to achieve the best
medical care possible.
you are found to have no antibodies against the disease, the nurse will immunise
you. When you become pregnant, the doctor will see you and he will refer you
to your local hospital booking clinic. We will always share your care with the
local hospital.
CERVICAL SMEARS
We invite all women between the ages of approximately 25 and 64 to attend for
smears.
FAMILY PLANNING
Please inform us of any change in your circumstances, such as a change of name,
surname, address or telephone number. Please ensure that we have your correct
telephone number.
The doctor can provide you with a full range of family planning care – the pill,
caps, coils and smears. If you want a coil fitted please book an appointment.
Follow-up clinics can be booked with the nurse.
If the Doctor agrees that a routine referral is necessary, this will be made and sent
to the hospital within one week. Urgent referral will be made within 24 hours
CHILD DEVELOPMENT
USE OF COMPUTERS AND PERSONAL HEALTH
INFORMATION
We are registered under the Data Protection Act and have robust systems in place
to protect your confidentiality.
We will examine children and discuss your child’s development during any surgery.
We also run a special child health surveillance clinic, when we offer 20 minute
appointments to see children by arrangement at 8 weeks, 8 months and 3 years. We
hope that you will use this clinic if you are concerned about your child’s behaviour
or development at any stage.
Acutely ill children should be brought to the surgery as usual.
Personal health information is used to monitor the practice’s screening activities.
Occasionally anonymous health information is sent to the PCT to monitor quality
standards and for pre & post payment verification purposes. Patient information
will always be kept strictly confidential.
SELF CHECK-IN
We have a touch screen located by the front reception desk. This allows patients
with appointments to check themselves in without waiting for a receptionist and
saves queuing. The screen by the reception desk is very simple to use and patient
confidentiality is assured. Simply follow the on-screen instructions (available in a
variety of languages) or ask a receptionist to show you show to use it.
VACCINATIONS AND IMMUNISATIONS
Government recommendations change from time to time.

Adults also require a tetanus and polio booster every 5-10 years. Many adults
born before 1961 have never had a basic course of tetanus and polio
immunisations. Please make an appointment with the nurse, we strongly urge you
to do so.
the NHS charges apply to all patients, including those exempt from prescription
charges, children and the elderly.
We also offer influenza and pneumococcal vaccinations. The influenza vaccination
is recommended for people over the age of 65 or who suffer from asthma, diabetes,
heart disease, liver disease, rheumatoid arthritis or other chronic illness. It is also
recommended for careers of elderly or disabled people
Please note that we are only able to accept payment by cash or cheque (supported by
a cheque guarantee card). We do not accept credit or debit cards. Payment is
expected on the day of the vaccination. Most vaccinations are held in stock so if you
book an appointment you will normally receive the vaccination on that day.
We administer influenza vaccinations each year from the first week in October.
Please call to book an appointment. Patients are entitled to receive a free influenza
vaccination if they:
 are 65 years or older
 live in a residential or nursing home
 suffer from a chronic heart or chest complaint
 suffer from chronic kidney disease
 have diabetes
 have lower immunity due to disease or treatment
 suffer from chronic liver disease
 are the main career for an elderly or disabled person whose welfare may be
at risk if they fall ill
We are also a Yellow Fever Centre authorised to administer yellow fever
vaccinations.
The pneumococcal vaccine is offered to all patients over the age of 65. It protects
against serious diseases that can be caused by the pneumococcus bacterium. These
include:
 Pneumonia
 Septicaemia
 Meneingitis
 Pericarditis
 Peritonitis
Pneumococcal pneumonia occurs in 1 in every 1000 people. Those most at risk are:
 those over the age of 65
 those with diabetes
 those with chronic heart, lung, liver or kidney disease
 those with immune deficiency due to diseases or treatment
 those who have had their spleen removed or a severe disorder of their spleen
A number of travel vaccinations are given free of charge. However there are some
vaccinations that incur a fee. For those travel vaccinations which are not available on
SICKNESS CERTIFICATES
A sickness certificate is not required for periods of illness lasting between 1-3 days;
simply explain your absence to your employer.
An SC1 form, available form your employer, should be completed for periods of
illness lasting between 3-7 days.
For periods of illness lasting longer than 7 days a sickness certificate may be
required from the doctor. Please book an appointment if this is required. This is in
accordance with the National Guidelines and aims to prevent patients making
appointments for minor self-limiting illness.
For certificates and letters outside of this criteria a charge may be incurred.
HELP US TO HELP YOU
You will be treated as an individual and will be given courtesy and respect at all
times, irrespective of your ethnic origin, religious belief, personal attributes or
the nature of your health problems. In return we ask you to appreciate that we
are all trying to satisfy the needs of all our patients and would appreciate your
co-operation.
Medical staff will advise you of the treatment that they think is appropriate for
and you and which you may discuss with them if you wish. No care or treatment
will be given without your informed consent.
You can see your health records written after 1 November 1991, subject to any
limitations in the law. Please ask at reception for an information leaflet, if
required.
