PART III-A – REQUIRED PROGRAM INFORMATION
Due Friday, April 15, 2011 by 12:00 p.m.
The Wraparound Zone Initiative is intended to strengthen district systems of support so that schools can
systematically address students’ non-academic barriers to learning in classrooms by connecting them
and their families with services that can address these barriers. The Initiative is also designed to improve
overall school climate by promoting the importance of social/emotional and physical health and wellbeing. While each participating district will implement the Initiative differently based on the range of
community partnerships and student needs, the district must ensure that the following Wraparound
Zone Functions will be performed:




Each participating school creates a climate and culture that promotes mental health and positive
social, emotional, and intellectual growth for students resulting in a new standard of practice
understood and practiced by every member of each school community;
Each participating school implements a proactive system of identifying student needs in key
academic and non-academic areas leading to both universal supports and targeted interventions;
Each participating school integrates a range of resources to tailor the student services from both
within the school and the larger community. The range of services includes prevention, enrichment,
early intervention, and intensive/crisis response services; and
Each participating district monitors school program effectiveness, creates district-wide supports for inschool social workers, and connects families to services by convening relevant agencies and
organizations.

Applications for the Wraparound Zone Initiative will be submitted in two main parts:
The initial application, due Friday, April 15, 2011, 12:00 p.m. will outline district capacity and provide an
initial view of how the district will implement a Wraparound Zone Initiative.
The second part of the application, required only by those districts that provide sufficiently compelling
capacity in the first application phase (to include a site visit/interview), will be due on Tuesday, May 31,
2011, 12:00 p.m. This will include a more detailed, comprehensive plan for the design and
implementation of the Wraparound Zone model. Further guidance will be offered to those districts
selected to participate in the second part of the application process.
For the initial application, responses to the following questions are due to the Department on April 15,
2011, along with a Part I signature page.

1. Fill in the Part I Signature Page and include the total amount of funds the district anticipates
requesting with the May 31, 2011 application ($16,000 per school, for a total of 3-10 schools).
2. Participating Schools: Identify 3-10 school(s) in the district that will be included in developing a
Wraparound Zone model. Indicate the following information about each school:
 School name and address;
 Year the school will begin participating in the Wraparound Zone Initiative (if phased rollout);
 Number and type of staff;
 Total number of students;
 Rationale for selecting school/Wraparound value: a concise analysis of how students and
families from the school will benefit from a Wraparound Zone model and what developing a
Wraparound Zone model will accomplish that the school feels it cannot or is not currently
doing (student demographics, trends related to student/family needs, assessment data);
 List of any partnerships the school currently has with agencies or organizations that provide
social/emotional/behavioral/physical services for students and families.

3. Previous Experience: Briefly describe any experience the district or participating schools have
with any of the following:
 Wraparound service models;
 Collaborative case management;
 Mapping school and community resources/services and developing a system for coordinating
referrals;
 Formal school culture/climate surveys (e.g., to assess student, teacher, and parent
perceptions of district and school practice);
 Using or developing social/emotional/behavioral health indicators.
4. Zone Initiative Design and Implementation (maximum 6 pages): Describe your proposed
approach for implementing the Wraparound Zone Initiative. Please speak directly, concisely, and
candidly to each of the areas listed below.
You do not need to provide lengthy narrative responses or a detailed plan for how exactly you will
implement the Zone Initiative. At this stage, proposal reviewers want enough information to
understand your methodology and priorities, as well as the specific advance your proposed
approach represents for student support at participating schools and in the district as a whole.
a) School Level Zone Activities: What will the key activities of the Wraparound strategy be each
year?
b) School Level Zone Coordination: Who will be responsible for leading, designing, and
implementing Wraparound activities at the school?
c) Zone Partners and Resources: What specific internal and external resources do you want to
involve in the Wraparound Initiative (district or school programs, external agency or
organization partners, state or national partners, etc.)? How involved are these programs and
organizations at participating schools now? Are there any partner/service gaps you need the
Wraparound Zone Initiative to address?
d) District Level Role and Activities: What is the role of district level staff in developing and
supporting the Wraparound Zone Initiative? Do you envision any district level Wraparound
Zone activities?
e) Barriers to Zone development or implementation: What challenges do you anticipate and how
do you plan to address them?
f) Quality Assurance/Assuring Impact: What is your basic plan for assessing successful
implementation of the Zone Initiative and the overall impact of the Initiative? How will lessons
learned be communicated and shared within and across the district?
g) Technical Assistance Support: What support do you need in order to successfully design and
implement your Wraparound Zone Initiative?
5. Integration and Sustainability: Describe the plan to continue performing the Wraparound Zone
Functions when the three-year grant expires.
6. Anticipated Expenses: Provide a rough budget outline or list of the types of expenses you will
incur in the design and implementation of the Wraparound Zone Initiative.
