Guysborough Antigonish Strait Health Authority Department of Mental Health Services POSITION DESCRIPTION POSITION TITLE: Wraparound Coordinator CLASSIFICATION HC-77 BARGAINING UNIT CUPE- HC Building Bridges of Support DEPARTMENT Mental Health DIVISION Community Health POSITION SCOPE: Reporting to the Mental Health Services Manager functionally, the Mental Health Management team generally and specifically to Wraparound Advisory Team, the Coordinator is primarily responsible for the co-ordination of service delivery with partnering agencies and the community for at risk youth and their families. The Coordinator will actively support and implement a wraparound philosophy in which services are individualized, strengths-based and youth/ family-centered. The Coordinator is responsible for ensuring compliance with acts, regulations, policies, standards, guidelines, mission and objectives of the Department of Health and Wellness - Province of Nova Scotia and the Guysborough Antigonish Strait Health Authority (GASHA). MAJOR RESPONSIBILITIES: 1. Utilizes wraparound processes to assess the individual youth and family’s strengths and needs. Main Activities: Provide assessment of a youth’s developmental assets, identifying their strengths. Assist the youth/ family in establishing realistic goals. Assist the youth/ family in identifying individuals from participating agencies as well as others who could form a support team; wrapping around the youth and family to assist them in meeting their goals. Recognize the cultural diversity of the community and incorporate this when planning and coordinating services. Advocate for the dignity and self-respect of clients. Promote the autonomy of clients and help them to express their individual needs and values to obtain appropriate information and services. Involve client/ family in decision-making and in taking responsibility for their health. 2. Collaborate with identified agencies under the Child/Youth Strategy as well as other community partners and organizations, as deemed appropriate, to assist clients in planning, implementing and evaluating strategies to meet their goals. Main Activities: Support the navigation of the services and agencies for clients and their families Serve as a liaison to participating agencies that may be sources of referrals and/or service delivery. Assist the Youth/ Family in developing a support plan in consultation with other members of the identified support team. Assist the client in implementing the support plan. Provide periodic review with the youth/ family and their support team to ensure that the support plan is being implemented, evaluated and updated. Recognize the need for crisis intervention and counseling in emergent or urgent situations and facilitate appropriate access/referral to these interventions, as necessary. Demonstrate timely and accurate documentation in the client’s health record in accordance with departmental standards of practice. Interpret and communicate social, psychological and environmental need of the client/ family to other members of the support team. Maintain an expected level of confidentiality at all times. Identify and approach community programs and services that may support the development and implementation of the individualized support plans. Advocate for programs and other resources to support a client’s individual needs. Safeguard the trust of clients/ families, that information learned in the context of a professional relationship, is shared outside the identified support team only with the youth/ families permission or as legally appropriate. Foster increased youth engagement in community agencies. 3. Professional Responsibilities. Main Activities: Act in a manner consistent with professional responsibilities. Articulate the role of Wraparound Co-ordinator to clients, families, other health care providers and the community. Demonstrate a high level of skill in interdisciplinary team building, conflict resolution, and collaboration. Maintain knowledge and enhance competencies relevant to the position of Wraparound Coordinator through professional development activities, peer review and other strategies. Facilitate and foster communication, collaboration and linkages between key stakeholders. Participate in various departmental, community and provincial meeting/ committee functions as assigned. Assist in policy/ process development and the strengthening of the Wraparound Service and/ or Child Youth Strategy as requested. 4. Other responsibilities Main Activities: To support and implement, with the Wraparound Advisory Team, processes for the management, quality control and ongoing evaluation of the Wraparound Service. Increase community awareness and capacity for supporting youth and their families in addressing key issues. Other duties as assigned. MINIMUM FORMAL EDUCATION AND EXPERIENCE: - Bachelor’s Degree in the Social Sciences (i.e. social work, psychology), Education or Health Professions. - Three (3) years successful professional experience acquired post-degree in a private or public agency serving children and youth.. - Professional experience in direct service delivery to youth and families with complex needs. - Experience working independently and providing leadership, coordination under the direction of an advisory group preferred. - Familiarity of community resources. CERTIFICATION/ REGISTRATION/ DESIGNATION A satisfactory criminal record check. A satisfactory child sexual abuse registry check. Must have a valid driver’s license. SPECIAL KNOWLEDGE & SKILLS: A commitment to the principles of a Wraparound philosophy. Ability to work with both formal and informal service systems and to build partnerships in the community. Ability to work flexible hours; evenings and week-ends. Knowledge of child and youth growth and development and ability to work with at-risk youth and their families. Professional experience in direct service delivery to youth and families with complex needs. Non-violent crisis intervention CPR, First Aid and WHMIS TYPE AND LEVEL OF SUPERVISION: Works under the direction of the manager of the Child, Youth and Family program and consults members of the Building Bridges of Support Implementation Team. Supervision of other staff is not a requirement of this position. WORKING CONDITIONS The following are conditions that an employee in this position may be exposed to. Physical Demands Moderate driving requires sitting for long periods of time. Environmental Conditions Exposure to household conditions such as dust, smoke and pets which could result in potential health risks. Mental Demands Exposure to emotionally disturbing experiences. S/he is expected to maintain a calm and professional demeanor and demonstrate compassionate care to the client, family and possibly other members of the health care team. Interactions with clients and their families can be complex which may lead to mental fatigue or stress. Demonstrate good stewardship in the identification, reporting & mitigation of risks. Annually access the training and education on patient, client and personal safety made available by GASHA.
