For the Parents, the things that need to be in order for your child to be baptized are: 1). You and/or your spouse must be a member of a Catholic Church. 2). I will need a letter from your Church with their seal on it mailed directly to me to verify this information. I cannot accept a fax due to the seal will not come through. You and/or your spouse must attend a Baptism Class either here or at another Catholic Church before the Baptism. If you attend a Baptism Class at another Catholic Church, then I would need written confirmation mailed directly to me with the Church’s seal on it stating that you and/or your spouse have attended a Baptismal Class in the past 18 months. There is a form for the parents, The Informational Family Form, to fill out when they meet with me or download it from the Web site and bring it the class. There is also a form for each of the Godparents to fill out as well. For the Catholic Godparent(s), it is the Sacramental Sponsorship Certificate. If the other Godparent is not Catholic, but is from a Christian Faith, they would fill out the Christian Witness Certificate. I can send the godparents’ their forms if their addresses are provided to me by e-mail. Please let me know which certificate each of the Godparents needs so that I can send them the correct one. You can also download the form from the Website. If you decide to have download the forms, please make sure when the Godparents fill out their correct certificate and that they have their Church stamp it with a seal or have their church write a letter with the stamp seal on it. Bring all the forms with you when we meet, we will have all the information that is needed in order to permanently set the Baptismal date. If you choose to down load the Godparents’ forms, please remember that there are 2 (two) sides for each form. Baptismal dates are NOT finalized until I have received ALL the paper work from everyone. (over) For the Godparents: the qualifications are for having both,: 1). one must be a man for the Godfather and one must be a woman for the Godmother. 2).One of them must be Catholic and is a member of a Catholic Church.
