Pick up a “College/Major(s)/ Minor/Degree Form 100 Steven or at our office Aubert Hall Suite 327 it’s
attached to this document.

Complete the top section, in other words fill in the date, current college, name, current major, social
security number. Sign your name and the date.

Bring the form to the Director of Native American Studies for him to complete and sign the Declaration
of Minor section. The Director will then turn the form over to her administrative assistant for further
processing.
