Complete the top section, in other words fill in the date, current college, name, current major, social security number. Sign your name and the date. Bring the form to the Director of Native American Studies for him to complete and sign the Declaration of Minor section. The Director will then turn the form over to her administrative assistant for further processing. Native American Studies Minor Work Sheet Name: The Native American Studies (NAS) minor involves a minimum of 18 credits of course work.
