Welcome to Moorside Clinic Online
We are a group family practice located in Downham on the Bromley/Lewisham borders. As a
group practice there is no restriction on which doctor you see. We provide a comprehensive
family doctor service. The practice comes within the Lewisham Primary Care Trust area and the
nearest hospital is University Hospital Lewisham. There is limited parking – a resident’s permit
is required. There is disabled access from the front and rear of the surgery

Between 6.30pm - 8.00am please call 020 8698 4774. A recorded message will tell you how to
contact our Out of Hours Service, SELDOC. Please ensure you have pen and paper to note down
the number to call.
Please do not ask to see a doctor out of hours unless you genuinely cannot wait until the surgery
re-opens.
You can also contact NHS Direct on 0845 46 47 for confidential healthcare advice 24 hours a
day.
Call 999 in an emergency. Chest pains and / or shortness of breath constitute an
emergency.
NHS Direct
In the case of urgent need when the practice is closed you can call NHS Direct
(www.nhsdirect.nhs.uk/) (0845 4647) to speak to a triage nurse. Your needs will be assessed and
advice offered or arrangements made for you to see a doctor.
Please do not ask to see a doctor out of hours unless you genuinely cannot wait until the surgery
re-opens.

ROUTINE APPOINTMENTS
Morning appointments – please call on 020 8698 4774 between 2pm-6.30pm THE DAY before
the appointment is required.
Evening appointments – these can be booked up to 2 weeks in advance at any time.
Please note: Only one person can be seen at each appointment and each appointment is for a
minium of 10 minutes - If you require longer then please tell the receptionist when booking.
Please inform Reception if you wish to cancel your appointment as soon as possible to allow the
slot to be allocated to someone in need and to reschedule your appointment. If you fail to attend
3 consecutive appointments you will be informed that you may be removed from our list.
EMERGENCY APPOINTMENTS
If you require urgent medical treatment and cannot wait for a booked routine appointment, you
may call on 020 8698 4774 between 8.00-10.30 Mon to Fri for an emergency appointment on the
same day.
Home Visits
If you are too ill to attend the surgery and need a home visit please ring on 020 698 4774 before
10.00am where possible. Please try to give details of your condition to the Receptionist to help
your GP assess the urgency of the visit.
A doctor or nurse may phone you back as it may be that your problem can be dealt with by
telephone advice, or that it would be more appropriate to send a nurse, or indeed arrange a
hospital attendance.
House visits are only available for patients who are housebound because of illness or disability.
Please remember that several patients can be seen in the practice in the time that it takes to make
one home visit. There are also better facilities for examining and treating patients at the Health
Centre.
PRESCRIPTIONS
Repeat prescription requests must be made in writing at least 48 hours before they are needed.
We do not accept requests over the telephone.
When you receive a prescription, the white page on the right hand side is where your repeat
medication is listed. Simply tick the items you require and place it in the box in reception.
If you have had medication and it is not listed as a repeat, please make an appointment to see the
GP for more.
You may post your requests and remember to enclose a stamped addressed envelope if you wish
us to return it to you by post.
Some local chemists provide a collection and delivery service. Please ask your local chemist for
details and inform us if they will be collecting your prescriptions on your behalf as we need your
consent to give them your prescription.
Private prescriptions cannot be converted into NHS prescriptions.
Prescriptions Charges and Exemptions
Extensive exemption and remission arrangements protect those likely to have difficulty in paying
charges (NHS prescription and dental charges, optical and hospital travel costs).
The NHS prescription charge is a flat-rate amount which successive Governments have thought
it reasonable to charge for those who can afford to pay for their medicines. Prescription
prepayment certificates (PPCs) offer real savings for people who need extensive medication.
NHS charges
From 1 April 2009, the charges are:



Prescription (per item): £7.65
12-month prepayment certificate (PPC): £104.00
3-month PPC: £29.10
If you will have to pay for four or more prescription items in three months, or more than 14 items
in 12 months, you may find it cheaper to buy a PPC. The charge for a single prescription item is
£7.65, whereas a three month PPC will cost you £29.10 and a 12 month PPC £104.00.



SMOKING CESSATION, ASTHMA, DIABETES, HYPERTENSION,
CONTRACEPTIVE SERVICES, CERVICAL SMEARS, TRAVEL
VACCINATIONS
Please make an appointment with our Practice Nurse for the above.
ANTE-NATAL (Weds 9.30-11.30am)
This is run by a Community Midwife. Please bring a urine sample with you.
BABY CLINIC (Alternate Weds 12.00 - 1.00)
This is run by the Health Visitor to carry out development checks with the duty doctor and to
give general advice. The Practice Nurse is available to give primary immunisations.
Please note this clinic is for well babies. If your baby is unwell please book an appointment to
see your GP.
PATIENTS OVER 75 YEARS OF AGE (By appt)
If you are aged 75 or over you will be invited to have a health check once a year in your home or
by appointment at the practice.
NON-NHS EXAMINATIONS (By appt)
E.g Pre-employment medicals, insurance reports, please make arrangements with the
Receptionist. A fee may be payable so please check this at the time of booking.
Some services provided by the Practice are not covered by the NHS. Fees may be charged for
some services including: Some travel vaccinations, medical reports, letters etc. You will be
advised if any charges are relevant to you.
COUNSELLOR (Friday 2.00-5.00pm)
Counselling sessions may be offered by referral from the GP. We have a highly qualified
counsellor, Mr Harry Bhairo who runs the sessions.
CITIZEN’S ADVICE (Alternate Mondays 10am-12noon)
We are able to offer half hour sessions with a Lewisham Citizen’s Advice Officer who advises
on matters relating to benefits, housing, pensions etc.
FLU VACCINATIONS
Flu clinics take place every year from October – January. The dates and times are advertised in
September/October.

At two months old:

Pneumococcal infection - pneumococcal conjugate vaccine (PCV) - one injection
At three months old:

Meningitis C (meningococcal group C) (MenC) - one injection

Travel Information
If you require any vaccinations relating to foreign travel you need to make an appointment with
the practice nurse to discuss your travel arrangements. This will include which countries and
areas within countries that you are visiting to determine what vaccinations are required. There is

It is important to make this initial appointment as early as possible - at least 6 weeks before you
travel - as a second appointment will be required with the practice nurse to actually receive the
vaccinations. These vaccines have to be ordered as they are not a stock vaccine. Your second
appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.
Some travel vaccines are ordered on a private prescription and these incur a charge over and
above the normal prescription charge. This is because not all travel vaccinations are included in
the services provided by the NHS.
Test Results
Test Results
Test results can be given between 11.00-12.00 noon daily by calling 020 8698 4774. It normally
takes about a week for the results to come back
Note that the practice has a strict policy regarding confidentiality and data protection and we will
only release test results to the person to whom they relate unless that person has given prior
permission for the release of this data or they are not capable of understanding the results.
When you take your test you will be told how long it will be before the results are returned to the
practice.
It is your responsibility to check your results and to make an appointment to discuss them with
your doctor if your are advised to do so.
Registration
REGISTRATION
The Practice offers NHS services to patients living within the catchment area. Please check with
a Receptionist that you live within this area before you begin registration.
You must live permanently within the area and will be asked for your NHS numer, and proof of
address e.g. utility bill. You will be asked to make an appointment with:
1. Our Assistant Practice Manager (Mrs Sandra Wright) for a registration interview and
also,
2. Our Practice Nurse (Gillian Jenkins) for a new patient health check for which you will
need to produce a urine sample.
A registration pack is available from Reception which includes a Checklist. You will need to
complete this Checklist in order to be registered with us.
Checklist
1. Completion of Form GMS1. It is essential that you complete this form fully as it goes to
the Primary Care Trust who will return it if it is incomplete and you will not be
registered.
2. Medical Card.
3. Babies. Please provide the red form FP58 which you are given when the baby is born.
4. If you are registering children under 16, please provide details of all past immunisations
which can be found in your Child Health Surveillance Book (the Red Book).
5. If you are female aged 18-65 please provide date of last cervical smear on the sheet
provided.
6. Reception will provide you with a specimen bottle and bag in your registration pack.
Please bring a urine sample to your new patient health check.
7. By signing the patient consent form you will give your GP authorisation to access
information from external agencies to enable them to provide you with the best possible
care.
Confidentiality
MEDICAL RECORDS AND CONFIDENTIALITY
All your medical history and information are kept in files and on the computer. Your medical
and personal information are strictly confidential and all staff are trained to maintain this
confidentiality.
The Data Protection Act 1998 allows you to access and obtain copies of your medical records.
The guidelines governing this stipulate that a £10 fee may be charged for access to your medical
records, a request form needs to be filled in (available from the Practice Manager) and there is a
charge for copies made up to a maximum of £50.00.

COMPLAINTS AND SUGGESTIONS
If you have any complaints, comments or suggestions regarding the service we provide please
put it in writing to the Practice Manager. We value any comments and suggestions that our
patients have.
