Ref Business Rule
1
The Diabetes Alert process must be used for
clients who have been diagnosed with
Diabetes.
 Intake will initiate the process
 It is the Diabetes Educator’s
responsibility to confirm the ‘Alert’
status and make necessary
amendments when the client’s
circumstances or diagnosis change.
Reason
Tagging clients with a ‘Diabetes
Diagnosis’ will enable you to analyse
the number and types of diabetes
diagnoses and the services people are
accessing at KCHS.
