introduced music in our waiting area and now have late night appointments with nurses. We have
also extended the care we provide by becoming the local hub for alcohol misuse, helping people
to overcome their alcohol addiction.</p>

We offer extended hours from 6.30-7.30pm on a Monday and Thursday.
Training Closures
The surgery will be closed for training on the third Thursday of each month between 12:30pm 16:30pm as the clinicians have protected learning time outside of the practice. Please note whilst

The Lister walk in centre is open from 8am - 8pm everyday including weekends and bank

When you call SELDOC a trained receptionist will take details from you. Please have the

NHS Direct (www.nhsdirect.nhs.uk/) on 0845 4647. A triage nurse will assess your situation and
take appropriate action. This could simply be by offering advice or where necessary by making
arrangements for you to be seen by a doctor.
Out-of-hours services are generally busy so please think carefully before asking to see a doctor
and only do so if you genuinely cannot wait until the surgery re-opens.
In a genuine emergency you should call 999. Chest pains and / or shortness of breath
constitute an emergency.

alt="circled_day" /></span></h2> <p>DMC Chadwick Road operates an appointment system
which ensures that patients can choose which of our clinicians they wish to see at a time that is
convenient. Patients can book up to 4 weeks in advance. You have the option of having a
telephone consultation with various members of the clinical team not just a GP. this will save
you time and it is not always necessary to attend the practice. We also offer a Telephone triage
Service every morning, you will be booked in with a Doctor to call you back, this are 5 minutes
duration <strong>Please note you must ensure that you are available to take to call once
booked</strong></p> <p>Appointments are for 10 minutes and within a 10 minute consultation
it is not always possible to deal with multiple health problems, it is at the GP's discretion to
identify whether a second problem can be dealt with in a consultation or whether it is more
beneficial for the patient to come back for a second appointment. However, if you feel your
problem may take longer please advise at the time of booking and you will be allocate a 20
minute slot depending on the availability. It is the aim of the practice to deliver best possible
care.</p> <p>Our GP’s consist of both full and part timers from Monday to Friday. We
also offer extend hours appointments on a Monday and Thursday from 6.30pm – 7.30pm
for various clinical team members i.e. GP’s Primary care pharmacists and a CPN (during
half terms). Alternative Saturday mornings (9-1pm) and late Monday evenings (59pm) for the midwife based at DMC Crystal Palace Road SE22 9EP. Please ask at
reception for further details.</p> <p>You can book appointments by:Coming in person to
the reception or calling us on: 0207 639 9622</p> <p><strong>Cancellations</strong></p>
<p>If you cannot attend an appointment for any reason please call us on 020 7639 9622 as soon
as possible in order for us to give the slot to someone else. Patients who continue to miss their
appointments may be removed from our practice list.</p> <p>In addition, We now have
various other Health Care Professions on site daily.</p> <p><strong>Primary Care Pharmacist
Team</strong> available Monday – Friday and offer the following:</p> <p>Spirometry,
Asthma, Smoking Cessation, Influenza Vaccines, NHS Health Checks, BP Monitoring,
Medication reviews, Emergency contraception, Diabetes reviews and Minor aliments advice</p>
<p><strong>Community Psychiatric Nurse</strong> available Monday – Thursday and
offer the following:</p> <p>Mental Health sessions focusing on Psychological strategies,
Medication reviews, relapse prevention, Benefits and Housing advice, Physical Health reviews,
Therapy and Psychiatry</p> <p>You may find that an appointment with one of our practice
nurses or Health Care Assistants will help you with your problem.</p> <p><strong>Practice
nurses</strong></p> <p>In a number of cases it might be worth considering an appointment
with a practice nurse or healthcare assistant (currently in training) rather than a doctor.
Practice nurses are qualified to deal with many ailments and you may be seen more quickly. Our
practice nurse offers late night appointments on a Monday and Thursday until
7.30pm</p> <p><strong>If you change address, name or telephone number, please let us know
as soon as possible so that we can update our records. You can inform us by visiting the surgery
with proof of address dated within 3 months and completing the appropriate form</strong></p>
<h2>Home Visits</h2> <p><img style="float: right; width: 186px; height: 173px; border-width:
0px;" src="images/homevisits1.jpg" alt="Doctor's bag" />We offer home visits for patients who
are unable to attend the surgery due to disability or serious illness. To request a home visit,
please call the surgery before 11:00 and the doctor will assess whether a home visit is
appropriate</p> <p>You can also be visited at home by a community nurse if you are referred
by your GP. You should also be visited at home by a health visitor if you have recently had a
baby or if you are newly registered with a GP and have a child under five years.</p>
<h2>Sickness Certificates</h2> <p>You do not require a doctor's sickness certificate for any
illness lasting seven days or less. Your employer may however require you to complete a selfcertification form (SC2) which is available from your employer or on the <a title="SC2 Form"
href="http://www.hmrc.gov.uk/forms/sc2.pdf" target="_blank">HMRC website</a>.</p>
<h3>Evidence that you are sick</h3> <p><img style="float: right;"
src="website/D00000/files/sicknote.jpg" alt="Sickness Certificates" />If you are sick for more
than seven days, your employer can ask you to give them some form of medical evidence to
support payment of SSP (statutory sick pay).</p> <p>It is up to your employer to decide whether
you are incapable of work. A medical certificate, now called a 'Statement of Fitness for
Work’ (see below) from your doctor is strong evidence that you are sick and would
normally be accepted, unless there is evidence to prove otherwise.</p> <p>You could also
provide evidence from someone who is not a medical practitioner, e.g. a dentist. Your employer
will decide whether or not this evidence is acceptable. If your employer has any doubts, they
may still ask for a medical certificate from your GP.</p> <h3>Statement of Fitness for Work ’Fit Note'</h3> <p>The 'fit note' was introduced on 6 April 2010. With your employer's
support, the note will help you return to work sooner by providing more information about the

<p>Thank you to all those patients who gave the practice feedback on this ongoing issue with
patients failing to attend booked appointments. We have now created a DNA policy  as
from 1.12.2014:</p> <p><strong>Introduction</strong></p> <p>As a practice we would like to
reduce the rate of DNAs by the use of education and by considering the removal of those who
persistently do not attend pre-arranged appointments. Approximately <strong>199</strong>
appointments per month are ‘Did Not Attend’ (DNA), i.e. the patient does not turn
up for the appointment and does not contact the surgery in advance to cancel/change
appointment. The affect of these are:</p> <ul> <li>An increase in the waiting time for
appointments</li> <li>Frustration for both staff and patients</li> <li>A waste of resources</li>
<li>A potential risk to the health of the patient</li> </ul> <p>Also see Removal of Patient from
List policy for up to date procedural guidelines.</p> <p><strong>GENERAL
Policy</strong></p> <p>If a patient fails to attend a pre-booked appointment on more than two
occasions in the last 12 months, an informal warning letter will be sent to the patient, advising
them that a further occurrence could risk removal from the practice.</p> <p>If the patient fails
to attend another appointment, the matter will be discussed at a Practice Meeting and a majority
agreement will be reached as to whether the patient will be removed from the practice list. In
which case a formal warning letter will be issued.</p> <p>Warning letters are valid for a period
of 12 months. Removal based on warnings greater than 12 months old will be invalid – in
this case a further formal warning and period of grace will be required.</p> <p>Admin team will
review all DNAs on a daily basis and send the first letter out the day after the DNA to all patients
who failed to turn up to a booked appointment at the practice.</p> <p>A report of frequent nonattenders will be produced monthly and reviewed by the Practice Manager and clinical team to
identify any patients who should be excluded from the general policy, for reasons such as
memory problems.</p> <p>Patients included in the monthly report will then be reviewed in a
Clinical Meeting and appropriate action on individual patients taken i.e. informal letter sent if
they are deemed to be patients who are persistently failing to turn up.</p> <p> </p>

Repeat Prescriptions
Patients whose long-term medication has been approved on a repeat prescription basis can reorder in a number of ways:



Tick the repeat prescription box on your current prescription and either post it to us or
drop it into our reception.

Online via the link at the top of this page. New users will need to complete a simple
registration prior to using the service.
You can also now contact your local chemist and hand in your request for a repeat prescription to
them. The pharmacist will process it for you and arrange collection. Your GP will still review
your medication as before, but this will save you from multiple trips to the GP.
We generally supply two months worth of medication.
Please note the following: We do not take repeat prescription requests over the telephone.
If you are prescribed a new medication from somewhere other than the surgery you will need
to see the doctor in person

The Electronic Prescription Service (EPS) is an NHS service. DMC Chadwick Road can send
your prescription electronically to the place you choose to get your medicines or appliances
from.
What does this mean for you?



If you get a repeat prescription, you will not have to spend your time going to your GP
practice each time to pick up your paper prescription. Instead your GP will send it
automatically to the place you choose, with no chance of it getting lost.
You will have more choice about where to get your medicines from, because they can be
collected from a pharmacy near to where you live, work or shop.
You may not have to wait as long for your prescription items, as there will be time to get
your repeat prescription items ready before you arrive. If any of the medicines on your
prescription are out of stock, they can be ordered in ready for you.
When will I be able to use EPS?

The prescription will be ready for collection 48 hours later. We generally supply two months
worth of medication as we feel that medication needs to be reviewed on a regular basis to ensure
that you are getting the appropriate treatment.
Prescriptions Charges and Exemptions
Extensive exemption and remission arrangements protect those likely to have difficulty in paying
charges (NHS prescription and dental charges, optical and hospital travel costs).
The NHS prescription charge is a flat-rate amount which successive Governments have thought
it reasonable to charge for those who can afford to pay for their medicines. Prescription
prepayment certificates (PPCs) offer real savings for people who need extensive medication.
NHS charges
These charges apply in England only. In Northern Ireland, Scotland and Wales
prescriptions are free of charge.



Prescription (per item): £7.65
12-month prepayment certificate (PPC): £104.00
3-month PPC: £29.10
If you will have to pay for four or more prescription items in three months, or more than 15 items
in 12 months, you may find it cheaper to buy a PPC.



Our practice nurses can help you with everything from long-term management of chronic
diseases to health promotion and screening.

Asthma & Diabetes
Monitoring asthma and diabetes to prevent complication and optimise medication.

Bloods
Routine blood tests.

Child Health
Monitoring child development and administering vaccination and immunisations for
babies.

Cholesterol Checking
We offer routine monitoring and a test to those with a family history of raised cholesterol.

Contraception
We can provide you with the full range of contraception, from free condoms to the
morning-after pill, as well as offering general sexual health advice and cervical screening
and STD testing.

Ear Care
We offer ear syringing to unblock wax and advise about preventing ear problems .

Health Assessment
We offer Well Woman and Well Man clinics and blood pressure checks.

Pregnancy Tests
We offer free pregnancy tests.

Sexual Health Advice
General sexual health advice, cervical screening and STD testing.

Counselling
Counselling for a wide range of patient needs.

Vaccinations and Immunisations
Child immunisations including immunisations for babies. We also do travel vaccinations,
but please ensure that you have your vaccinations at least six weeks prior to travel.

Wound Dressings and Wound Care
We can replace dressings and provide general advice and care to optimise healing. Stitch
and suture removal.
Clinics
Our Clinics

Audiology
InHealth run a clinic on Monday, Tuesday, Wednesday and Thursday to assess the
hearing of adults. This is managed by InHealth and referrals are direct from the GP.

Baby Clinic
We run a baby clinic on a Tuesday from 13:30 – 15:30. This is a drop-in clinic for advice
and weight checking. Practice nurses are also available, so please ask our receptionist.

Cognitive Behavioural Therapy
We can help treat a variety of problems including anxiety, eating disorders and substance
abuse. We run high and low intensity sessions every Tuesday.

Prevention of Heart Disease
All patients with heart disease will receive annual monitoring. We have access to health
trainers who can provide healthy lifestyle advice. We are also starting to do vascular
checks on all 40-70 year olds. Please book in with the practice nurse for further advice.

Maternity Services
There is a midwife practice at the surgery for antenatal, home birth and postnatal care.
We run sessions on Monday, Friday and Saturday mornings at the Crystal Palace Road

Minor Surgery
The minor surgery clinic is run for the excisions of moles, cysts and lumps.
Smoking Cessation Advice
To book an appointment with one of our specially trained smoking advisors, please call
the surgery or ask at reception.

Travel Health Clinic
Any vaccinations can be arranged through our practice nurse. Please come in to see the
nurse at least six weeks prior to travel. You can then discuss your vaccination needs and
be advised of any costs involved. We can also immunise you against Yellow Fever
without you having to visit a specialist clinic. Fee of £65 per patient applies.

Weight / Diet
General advice on diet and healthy nutrition is available from the practice nurse. If
necessary you will be referred to by the community dietician who is available during
surgery hours on Tuesdays, Wednesdays and alternate Thursdays. We have a
height/weight and blood pressure machine situated in reception for all patient to use,
please ask at reception if you require further details.
Non-NHS Services
Non-NHS Services
Some services provided are not covered under our contract with the NHS and therefore attract
charges. Examples include the following:





Medicals for pre-employment, sports and driving requirements (HGV, PSV etc.)
Insurance claim forms
Prescriptions for taking medication abroad
Private sick notes
Vaccination certificates
The fees charged are based on the British Medical Association (BMA) suggested scales and our
reception staff will be happy to advise you about them along with appointment availability.
Test Results
Test Results
You do not need to come in to get your results
We will either call you or send a letter to you to advise you on what to do in the event of
abnormal test results. If the result is abnormal, we will call or write to you in person to advise
you to book either a Telephone consultation or an appointment to see the Doctor/Practice Nurse.
However, you can find out your results by ringing us on 020 7639 9622 after 15:00.
Please note that all results need to be checked by the doctor before they can be given out.
The doctor will advise if any further action is needed following the test after they have
checked the result.
Most blood tests will take around three days to reach the surgery after you have had the test.
Urine results will take around seven days to reach the surgery, while X-rays and scans can take
up to two weeks for the results to come back to the surgery
Please note that we do have a strict policy regarding confidentiality and data protection. In this
respect we will only give out results to the person they relate to unless that person has given prior
permission for their release or if they are not capable of understanding them.
Blood Tests
Blood Tests
A blood test is when a sample of blood is taken for testing in a laboratory. Blood tests have a
wide range of uses and are one of the most common types of medical test. For example, a blood
test can be used to:



assess your general state of health
confirm the presence of a bacterial or viral infection
see how well certain organs, such as the liver and kidneys, are functioning
A blood test usually involves the phlebotomist taking a blood sample from a blood vessel in your
arm. and the usual place for a sample is the inside of the elbow or wrist, where the veins are
relatively close to the surface. Blood samples from children are most commonly taken from the
back of the hand. The child's hand will be anaesthetised (numbed) with a special cream before
the sample is taken.

X-Ray
X-Ray
An X-ray is a widely used diagnostic test to examine the inside of the body. X-rays are a very
effective way of detecting problems with bones, such as fractures. They can also often identify
problems with soft tissue, such as pneumonia or breast cancer.
If you have a X-ray, you will be asked to lie on a table or stand against a surface so that the part
of your body being X-rayed is between the X-ray tube and the photographic plate.
An X-ray is usually carried out by a radiographer, a healthcare professional who specialises in
using imaging technology, such as X-rays and ultrasound scanners.

Registration
New Patient Registration
If you live within our practice area you are welcome to register with us and our reception staff
will be happy to guide you through the process.
You will need to complete a registration form (GMS1) and a health questionnaire which will
provide useful information whilst we wait for your medical records to arrive from your previous
doctor.
All new patients are offered a health check with a member of the healthcare team to ensure that
any required tests are up to date and that we have an accurate note of any repeat medication you
may be taking.
Medical treatment is available from the date of registration. Please contact reception for further
information.

Registering Online
Online 'Pre-Registration' With The Practice
If you wish to pre-register click on the link below to open the form. When you have completed
all of the details, click on the "Send" button to mail your form to us. When you visit the surgery
for the first time you will be asked to sign the form to confirm that the details are correct.
Pre-registration Form
When you register you will also be asked to fill out a medical questionnaire. This is because it
can take a considerable time for us to receive your medical records. There is an online version of
this file too, which you may fill out and send to us. When you come to the surgery you will be
asked to sign this form to confirm that the details are correct.
Online Medical Questionnaire For New Patients
Note that by sending the form you will be transmitting information about your self across the
Internet and although every effort is made to keep this information secure, no guarantee can be
offered in this respect.
Alternatively you may print off a registration form, fill it out and bring it in with you on your
first visit to the practice.

Temporary Residents
Temporary Registration
If you are ill while away from home or if you are not registered with a doctor but need to see one
you can receive emergency treatment from the local GP practice for 14 days. After 14 days you
will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on
the local practice list and still remain a patient of your permanent GP. After three months you
will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do
not have to accept you as a temporary patient although they do have an obligation to offer
emergency treatment. You cannot register as a temporary patient at a practice in the town or area
where you are already registered.

Practice Policies
Confidentiality & Medical Records
The practice complies with data protection and access to medical records legislation. Identifiable
information about you will be shared with others in the following circumstances:



To provide further medical treatment for you e.g. from district nurses and hospital
services.
To help you get other services e.g. from the social work department. This requires your
consent.
When we have a duty to others e.g. in child protection cases anonymised patient
information will also be used at local and national level to help the Health Board and
Government plan services e.g. for diabetic care.
If you do not wish anonymous information about you to be used in such a way, please let us
know.
Reception and administration staff require access to your medical records in order to do their
jobs. These members of staff are bound by the same rules of confidentiality as the medical staff.
Freedom of Information
Information about the General Practioners and the practice required for disclosure under this act
can be made available to the public. All requests for such information should be made to the
practice manager.
Access to Records
In accordance with the Data Protection Act 1998 and Access to Health Records Act, patients
may request to see their medical records. Such requests should be made through the practice
manager and may be subject to an administration charge. No information will be released
without the patient consent unless we are legally obliged to do so.
Complaints
We make every effort to give the best service possible to everyone who attends our practice.
However, we are aware that things can go wrong resulting in a patient feeling that they have a
genuine cause for complaint. If this is so, we would wish for the matter to be settled as quickly,
and as amicably, as possible.
To pursue a complaint please contact the practice manager who will deal with your concerns
appropriately. Further written information is available regarding the complaints procedure from
reception.
Violence Policy
The NHS operate a zero tolerance policy with regard to violence and abuse and the practice has
the right to remove violent patients from the list with immediate effect in order to safeguard
practice staff, patients and other persons. Violence in this context includes actual or threatened
physical violence or verbal abuse which leads to fear for a person’s safety. In this situation we
will notify the patient in writing of their removal from the list and record in the patient’s medical
records the fact of the removal and the circumstances leading to it.

Kate recently joined the practice in July 2014 she is a fully quaified
Commuinty Psychiatric Nurse working in Primary Care.
She will be available on Monday - Thursday and will cover for
following servcies: will offer extended hours during half terms
Mental health sessions focusing on psychological strategies,
medication reviews, relapse prevention, benefits and housing advice,
physical health reviews, therapy and psychiatry referrals
Healthcare Assistants

Marsh
Healthcare assistants support practice nurses with their daily work and
carry out tasks such as phlebotomy (drawing blood), blood pressure
measurement and new patient checks. They may act as a chaperone when a
patient or doctor requests one. Alison is currently in the process of
completing all her training.
Practice Management
Lyza Edney
The practice manager is involved in managing all of the business aspects of the
practice such as making sure that the right systems are in place to provide a high
quality of patient care, human resources, finance, patient safety, premises and
equipment and information technology. The practice manager supports the GPs and
other medical professionals with delivering patient services and also helps to
develop extended services to enhance patient care.
Reception
Receptionists provide an important link for patients with the practice and are your
initial contact point for general enquiries. They can provide basic information on
services and results and direct you to the right person depending on your health
issue or query. Receptionists make most of the patient appointments with the GPs
and nurses. They also perform other important tasks such as issuing repeat
prescriptions and dealing with prescription enquiries, dealing with financial claims,
dealing with patient records and carrying out searches and practice audits.

Medication queries and reviews, Hypertension, COPD and Spirometry,

monitoring & Minor aliments advice.

Comminity Midwifes - Based at Kings College Hospital, the midwife is with us
every Friday morning.
Health Visitors
A health visitor is a registered nurse who has received training particularly related
to babies, children and pregnant women. Their role is to provide families with
children under five years old with support and advice around the general aspects of
mental, physical and social wellbeing.
We currently have a baby clinic every Tuesday afternoon from 1.30-3pm
