No fax registrations accepted. Do not send registration via Interoffice mail.
If mailed-in registration fee is not received by October 26, for any reason,
payment will be required on-site.
Registration fees, if attending both days, include symposium, continental breakfast,
and lunch.
Because of space limitations, no guests will be allowed to join us for lunch.
The Friday night reception fee is $10 for registered attendees and $20 for guests, and
space is limited; refund policy below applies to reception.
Please make checks payable to Kaiser Permanente.
Refunds will be granted until October 25; no refunds after that date. Check payment
will be refunded (less the $10 processing fee) in the form of a voucher (restrictions
apply) for another symposium. Website registrations will be refunded to the credit

of clinical and professional topics for nurse practitioners, physician assistants, certified nurse-midwives,
registered nurses and other colleagues.

Friday or Saturday: parking in the back Porte Cochere $10 valet parking each day.
