Sons and Daughters of Virginia Founding Fathers Instructions for Preparation of Lineage Papers 1. Please do not submit your application for membership until you have received an invitation to join. The Membership Inquiry form, posted on the website, is available for inquires. 2. The application must be typed and completed in the space provided. Please do not attach documents to the application. Only one copy of the application and one set of documentation are required. 3. Names should be written in full, including maiden names of wives, if known. State the male first in each generation. The line of descent may be emphasized by capitalizing the last name of that ancestor in each generation. 4. Include dates of birth, marriage and death in the following format: 4 Oct 1633. 5. If information is unknown or not applicable, leave the space empty. 6. The proof of each generation must be listed on the lineage paper by document and volume (title and applicable page numbers). Documents providing proof of each generation/event are required. Pertinent information should be underlined in red on each proof for easy verification. 7. Verifiable sources may include: birth, death and marriage certificates; federal and state census records from 1850 or later; tombstone transcriptions and photographs (with the name and location of cemetery stated on reverse of photograph); Bible records (with copyright page included), published town histories and vital records; published family genealogies IF sources are cited. A copy of the title page from all sources must be provided. Record copies from other lineage societies are NOT acceptable. 8. Submission of SDVAFF Lineage Papers 1. One copy of the completed application should be mailed with all proofs to the Genealogist General at the address below. 2. If the original application is not approved by the Genealogist, an alternate pedigree may be submitted within one year. 3. Life Membership and Application Fee + Genealogist’s Fee - $360.00 This includes a certificate. The Application and Genealogist Fees are nonrefundable. 2. Supplemental applications may be submitted for a non-refundable fee of $40.00 each. Certificates for supplementals may be purchased for $10.00.
