2014 4-H ARTS CAMP YOUTH STAFF
Be part of the staff! We're looking for 9th through 12th graders (in the fall of 2014) to be part of the Arts Camp
State Staff. Youth staff are required to be available for planning with other Arts Camp staff members by mail,
by phone, and to participate in on-site sessions, August 22-23 and Friday night, October 3, at Camp Upham
Woods. Camp dates: Saturday, October 4 – Sunday, October 5. Upon selection there is a $75.00 fee.

1. List the arts projects and activities (theater arts, visual arts, photography, communication, and music and any
other arts activity) that you have taken in 4-H and the years in each project.

5. Write a paragraph to a page describing an arts event or activity which you have led (or one that you will
lead). Include the following: 1) goals or purposes, 2) description of participants (age, number), 3) length of
event, 4) planning, materials, and process, 5) preparation needed, and 6) how it concluded and what you felt
about it (what went well and what went wrong).
OTHER APPLICATION MATERIALS NEEDED:
1. Complete/update health form in 4-H OnLine at http://wi.4honline.com and submit with your application.
2. Two letters of recommendation must accompany this application: one from your 1) 4-H Youth
Development Agent; and the second from either your 2) 4-H Club Leader or school teacher or work
experience supervisor. (Returning Counselors only need letter of recommendation from Agent.)
