Gestational Diabetes Self-Management Education Program
Welcome
Welcome to the Vanderbilt Eskind Gestational Diabetes Clinic. We provide
comprehensive services to include gestational diabetes self-management education
instruction to help guide you in managing your condition and promoting the best
outcomes for your baby.
Clinic Information
The main clinic telephone number is 343-8332(VEDC). Your gestational diabetes care
will be managed and provided by Dr. Shubhada Jagasia. Your education training will be
provided by Patricia Patterson, RN CDE and Jennifer Garland, RD. The contact person
for the gestational diabetes program and any questions about your care is Kim Garton,
Administrative Assistant.
Your first visit may last up to 3 hours and includes comprehensive assessment by our
certified diabetes nurse educator and registered dietician. A follow up visit will be
scheduled one week after the initial visit. Your appointments schedule will be customized
based on the team’s findings during the first two visits. The final visit will occur 6 weeks
after you have your baby.

Should you have any persistent abnormal blood sugars please notify Kristyn
Wilson at 615-322-7719. After 4:00 pm, you may call the on-call physician at
615-343-8332.
Please check blood sugars before and 2 hours after completion of each meal
during the day (Breakfast, lunch, dinner).
Blood sugars before meals should be below 95mg/dl. Blood sugars 2 hours
after meals should be below 120mg/dl. If you are unable to check at 2 hours
after meals, please check at 1 hour after which should be below 140mg/dl.
Please keep a log of all your blood sugar readings (log will be provided) and
bring with you to every appointment. Please provide further input regarding
abnormal blood glucoses.
You will receive a “kit” or “bag” at your first visit that will contain
information and resources for managing your gestational diabetes care. Please
give your pharmacy phone number to the educator so we may call in
necessary prescriptions for you.
