CHAMPS GRANT APPLICATION CHECKLIST
Deadline: Must be postmarked on or before January 24, 2014.
Your application must be completed correctly and all requested materials must be submitted before the deadline to be considered
for funding. Incomplete applications, missing information or an absence of required attachments will disqualify your funding request
immediately. Faxed or e-mailed applications will not be accepted. There are no exceptions.

Upload of the completed CHAMPS budget page for proposed program

Upload Cover letter (1 page), which highlights the program, target group and dollar request

Upload Proof of tax-exempt status (IRS Determination letter or other)
(Not necessary for churches or public/private schools)

Upload Statement verifying use of alternate program site
(If program is offered at an alternative site other than organization’s operating locale)

Upload Letter(s) of agreement if collaborating with other organizations

Upload copy of the most recent audited financial statement (if available)
or your most recent unauditied financial statement

Upload copy of the organizational fact sheet/brochure
