Dean or Chair Checklist for Faculty Parental and Maternity Leaves
Please consider the following information when a faculty member requests maternity and/or
parental leave. Faculty may apply for one or both types of leave, provided they meet the criteria
required (as specified in the Syracuse University Faculty Leave Benefits policy).

Parental Leave
1.
Decide upon a mutually agreeable
timeframe for requested leave.
2.
If the faculty member is untenured,
determine whether s/he wants to extend
his/her tenure probationary period by
one year.
3.
Certify your faculty member is eligible
to receive the parental leave benefit by
jointly reviewing and signing the
“Faculty Parental Leave” form.

Confer with your faculty member on
approximate dates of maternity leave at
least 5 months in advance of the
expected birth or adoption date (if not
possible, the faculty member is
requested to notify within 5 weeks of
becoming aware).

Instruct faculty member to contact the
Human Resources Leave of Absence
Specialist at 443-5367.

Send the completed “Faculty Parental
Leave” form that your Dean has signed
to the Associate Provost (518 Crouse
Hinds Hall) for approval.

The Human Resources Leave of
Absence Specialist will send the faculty
member the “Application for
Disability/Maternity Benefits” that will
need to be completed by both the faculty
member and the faculty member’s
attending physician. This form needs to
be returned to HR anytime prior to the
leave or within the first two weeks of the
leave.
If the faculty members desires, s/he may
write a memo to her Chair requesting
that her tenure probationary period be
extended by one academic year. Send
the request along with the dean’s
concurrence to the Associate Provost
(518 Crouse Hinds Hall) for final
approval.
Combination of
Parental and Maternity Leaves
1.
Confer with your faculty member on

possible, the faculty member is

for the leave.

If the faculty member is untenured,

4.
Send the completed “Faculty Parental
Leave” form that your Dean has signed
to the Associate Provost for approval.

The Leave of Absence Specialist will
send the faculty member the
“Application for Disability/Maternity
Benefits” that will need to be completed
by both the faculty member and the
faculty member’s attending physician.
This form needs to be returned to HR
anytime prior to the leave or within the
first two weeks of the leave.
A. OBTAIN CHILD’S DATE (OR ANTICIPATED DATE) OF BIRTH OR ADOPTION
B. AGREE UPON TERMS FOR FACULTY MEMBER’S ABSENCE
- ½ semester without formal job duties
- 50% reduction in workload
- Full semester release from teaching duties (combined maternity and parental leave)
C. COMPLETE AND SIGN THE APPROPRIATE FORM(S)
