Acute Medicine
Locum Appointment for Service (LAS)
Full time
We welcome applications for a Locum Appointment for Service in Acute Medicine to join
our department. This post is available immediately until 4th August 2015.
This post is based in Forth Valley Royal Hospital, Larbert which is a state of the art
modern Acute Hospital. The successful candidate will join our team of five consultants
and other doctors in training. Candidates will be expected to work within the Acute
Medicine team and provide support for emergency admissions and ward patients. The
post will enable the post holder to gain excellent exposure to a full range of core
medicine.
NHS Forth Valley provides acute services for a population of 280,000 people in Central
Scotland. Acute services were centralised onto a single site at Forth Valley Royal
Hospital in July 2011. Forth Valley is a particularly attractive area in which to live and
work with a wide range of housing, excellent state and private schooling and extensive
sport and leisure facilities. Falkirk and Stirling have good motorway and railway links to
the nearby centres of both Edinburgh and Glasgow while the Scottish mountains are a
short distance to the north.

INTRODUCTION TO NHS FORTH VALLEY
NHS Forth Valley is a single, integrated healthcare system comprising acute care facilities and
community based services, which are delivered through Forth Valley Community Health
Partnerships.
The Board’s vision of improving health and healthcare for the people of Forth Valley is
underpinned by its Integrated Healthcare Strategy. The principles upon which this strategy is
founded have led to significant change and development in recent years. With further service
reconfiguration planned, this is an exciting time to work with NHS Forth Valley.
NHS Forth Valley is the largest employer in the area, with approximately 8000 staff from a wide
range of professional and support occupations. Currently, services are provided in one acute
hospital, four community hospitals and fifty-six health centres. Acute Services are based in
purpose built 860 bedded Forth Valley Royal Hospital, which opened in 2011 and provides
inpatient and outpatient services for a population of approximately 300,000.
THE FORTH VALLEY AREA
The working environment is excellent, as is the quality of life that Forth Valley has to offer them
and their families. The area has stunning scenery, such as the Trossachs and Loch Lomond. It
is steeped in history with Callander House in Falkirk, Stirling Castle, Bannockburn and the
Wallace Monument to name but a few of the sites in the area. It also has many cultural and
leisure facilities such as the Falkirk Wheel (the largest canal barge wheel in the world), the
MacRobert Arts Centre at Stirling and the National Sports Academy at Stirling University.
Forth Valley also lies equidistant between Scotland’s two largest cities; Edinburgh and Glasgow.
With their respective airports, both are little over half an hour from most parts of Forth Valley, on
the Falkirk, Larbert and Stirling area lines.
House hunters in the Forth Valley area are spoiled for choice. On offer is a large array of
established and new properties in a variety of attractive settings.
Education in the area is first class, with excellent provision of pre-school, primary and secondary
education. State schools in the area have strong reputations and there is also easy access to the
area’s private schools in Bridge of Allan, Dollar and Crieff.
On the further education front, Stirling boasts an excellent University and access to the University
cities of Glasgow and Edinburgh is easy. There are also further education Colleges in Falkirk,
Stirling and Alloa.
In addition to its scenery, history, culture and shopping facilities, residents in Forth Valley have on
their doorsteps a varied range of leisure and sporting facilities. There are marvellous golf
courses, hills and mountains and everything from sailing and windsurfing.

Acute Medicine – Infrastructure
The Acute Medical Unit consists of:
 Clinical Assessment Unit, (CAU)
 Acute Assessment Unit (AAU)
 Chest Pain Assessment Unit (CPAU)
This LAS post will be based working in the Clinical Assessment Unit from 1000-1800, Monday Friday
Clinical Assessment Unit (CAU)
This area (which receives up to 40 GP referrals per day to Medicine and Ageing and Health) has
16 trolleys/beds and 4 chairs.
The area benefits from a highly skilled and enthusiastic nursing team, led by charge nurse Norma
Pringle, who strongly supports the on call teams working in this area,
Nursing triage includes point of entry severity scoring using a pilot of the National Early Warning
Score (NEWS). Electrocardiography, plain film radiography and venesection or cannulation are
also undertaken where required prior to medical review.
The CAU LAS doctor is an important member of staff, and crucial to the smooth running of the
unit. The duties incude
 Clerking of new patients referred to General Medicine or Ageing and Health
 Practical procedures
 Promptly reviewing unwell patients, or those triggering NEWS
 Ensuring discharge letters are completed in a timely fashion
 Participating in ward safety meetings and morbidity/mortality meetings
 Although this is a LAS post, there are plenty of opportunities to get involved with quality
improvement, and attend teaching (where time allows)
 You will be working under the supervision of a Consultant Acute Physician
Acute Medicine at NHS Forth Valley
The unit was the first in Scotland to introduce the new National Early Warning System (NEWS),
which having been successfully piloted in AAU, has been introduced across all acute services in
NHS Forth Valley.
We benefit hugely from a highly skilled and motivated IT support team, who are working
cooperatively with us to develop innovative tools to support capacity planning and patient
tracking. These include an electronic handover system, touch-sensitive smart boards, and a
capacity dashboard tool.
We are actively involved in Patient Safety and Improvement initiatives in areas such as cardiac
arrest reduction, reliable recognition and rescue of deteriorating patients, medicines
reconciliation, and the development of care bundles for common pathologies presenting to acute
medicine. The NHS Forth Valley AAU/CAU structured approach to the deteriorating patient has
recently been adapted as the national approach, and published in the journal BMJ Qual Saf.
There are also regular Acute Medicine Morbidity and Mortality meetings, and the Acute
Physicians lead the hospital wide Morbidity and Mortality meetings.

The consultant Acute Physicians are also involved in a range of work related to proactive
capacity planning, winter pressures and escalation policies.
Finally the Acute Physician team are developing Ambulatory Care services for a range of
appropriate conditions in line with national best practice

The Acute Care Team is supported by an excellent Fastrack Occupational and Physiotherapy
Assessment Team, lead by Kenneth Slater. We are also fortunate to have a highly motivated unit
pharmacy team.
This core team is supplemented by “on call” teams from Medicine, Ageing and Health, Surgery
and Surgical Specialties. The Medical Specialties - cardiology, respiratory, gastroenterology and
endocrinology each provide a subspecialty “Physician of the Week” service Mon-Fri 0900-1700.
Patients are allocated to medical specialty within the Acute Medicine Unit and specialty teams
visit the unit daily to pick up these patients.
The libraries contain:
 current medical text books for the major specialties
 medical journals on subscription
 a comprehensive stock of teaching
 Internet access
 portable colour laptop PC for use with overhead projection
There are a comprehensive clinical skills teaching suite for the teaching of resuscitation
techniques and other procedural skills. The Scottish Simulation Centre is located within the
hospital. There are weekly postgraduate meetings involving all specialties. There are regular
evening postgraduate medical meetings in the hospital for both hospital and primary care medical
staff. Glasgow and Edinburgh with their Royal Colleges, University medical schools and
postgraduate facilities are within easy reach.

CRITERIA FOR THE POST







Full General Medical Council Registration
Completion of FY1 year

CONTRACTUAL DETAILS
Contracted activities for this appointment will be defined and agreed with the post holder in
discussion with the Lead Consultant of the Acute Medical Unit
The post holder will be professionally responsible to the Associate Medical Director for the
Medical Unit and managerially responsible to the General Manager of the Medical Unit.
The posts will be based in Forth Valley Royal Hospital.
The LAS doctor post is available on a full-time basis but the Board welcomes applications from
individuals who are unable to undertake a full time commitment and who may wish to work on
either a part-time or job share basis. The appointment will be subject to the new terms and
conditions of service for Doctors in Training.
The successful candidate will be required to undergo satisfactory pre-employment screening
before a formal appointment may be made.
The employment is subject to one calendar month’s written notice on either side.
Annual leave entitlement is 5 weeks per year, rising to 6 weeks per annum, dependant on salary
scale point with additional statutory holiday entitlement.

Pre-Employment Screening

Criminal Convictions - Protecting Vulnerable Groups (PVG) Scheme
The Protecting Vulnerable Groups (PVG) scheme has replaced previously disclosure arrangements
for people who work with vulnerable groups. The duties of this post require you to undertake
regulated work with children and young people (individuals aged under 18 years) and / or protected
adults (individuals aged 16 or over who is provided with (and thus receives) a type of care, support
or welfare service).
The post holder will be required to be a member of the PVG Scheme and to consent to NHS Forth
Valley obtaining a Scheme Record or Scheme Record Update, as appropriate. Any failure to
disclose convictions could result in the withdrawal of an offer of employment.

Health Screening

This post involves the performance of exposure prone procedures and therefore the successful
applicant will be required to produce evidence of their Hepatitis B and C Immunity Status. The
successful candidate will be required to produce evidence of Hepatitis B and C status prior to
appointment and to undergo satisfactory health screening. Any offer of employment will be
subject to satisfactory Occupational Health assessment.
