GYN BLADDER RETRAINING & SUPRAPUBIC CATHETER CARE DISCHARGE INSTRUCTIONS PLEASE PLACE PATIENT LABEL CAREFULLY HERE Physician Name:____________________ Physician Phone Number:________________ Re-clamp the catheter tube for another 2 The following instructions are for bladder hours, or when your bladder feels full (if retraining after your surgery if you are sent before the 2 hours have passed) home with a suprapubic catheter: You (or a caregiver) will be: Clamping and releasing the catheter at intervals. Attempting to urinate normally after your catheter has been clamped for a while. Recording the amount of urine you are voiding as well as any urine that remains in your bladder after a normal voiding. When recording amounts of urine, use ounces or milliliters. The word “voided” means the same as “urinated”. The word “residual” is the amount of urine that is left in your bladder after you voided. Begin by clamping the tube to your catheter securely. When your bladder feels full, or after 2 hours have passed: Place a urine collection pan on your toilet. You may use the one that was used in the hospital. Try to urinate while the catheter tube remains clamped. If you are able to urinate: Record the amount of urine under the “Voided” column on the form provided to you. Empty this urine into the toilet & flush. Immediately release your catheter clamp & allow the urine remaining in your bladder to drain into the catheter bag for approximately 10 minutes. Empty the urine from the bag into the collection pan & record the amount of urine under the “Residual” column on the form provided to you. You can then flush this urine in the toilet as well. If you are not able to urinate and/or you become uncomfortable: Release the catheter clamp and allow your urine to drain into the bag. Record “0” for the amount voided on the form provided. On your first return visit to our office: Bring the form with all of your voiding and residual records for your physician to review them. While at home, if at any time your bladder becomes uncomfortably full, and you are not able to void or drain your urine into the catheter bag: Please call your physician or Go to the Emergency Room. The following instructions are for suprapubic catheter site care: Clean the insertion site of the suprapubic site at least once a day and more often if your physician tells you to do so. Wash your hands and maintain privacy. Hold the catheter erect while cleaning. Use gauze moistened with cleansing agent ordered per physician to clean site. Swab in a circular motion starting closest to the drain and continuing in outward widening circles for approximately 2 inches. Use a new piece of moistened gauze to gently clean the base of the catheter, moving up and away from site of insertion. Do not pull catheter. Apply a dry split gauze around catheter around the insertion site and tape in place. Secure catheter to abdomen with tape or tube holder to reduce tension on insertion site. Check the bag and tubing for security. If you have any problems or questions, please contact your physician. I have received a copy of the above instructions. I understand and accept them.
