The post is for 5 sessions per week. This is a replacement Consultant post within the State Hospital reflecting the organisation’s commitment to the further development of psychotherapy services and the post holder will have a key role in guiding service development in psychotherapy in a forensic setting. They will also provide consultation and advice to other colleagues across the State Hospital and where appropriate to other services. The role of all Consultant Psychotherapists also includes a key input to training in line with guidance from NHS Education Scotland and the Royal College of Psychiatrists. It is anticipated that one Programmed Activity will be allocated to teaching and training with the remaining time to undertake assessment and treatment and associated professional activities. Identification and preparation of patients likely to benefit from short and longer term intervention (which may be provided directly by the post holder or by trainees or other professionals under supervision), Direct clinical interventions for male forensic patients including group psychotherapy, mentalization based therapy, psychodynamic assessment contributing to multi-disciplinary assessment and contribution to case based discussions and clinical management plans. To undertake consultation and liaison in both individual and group work. Clinical leadership in order to continuously improve access to and provision of psychotherapeutic approaches within the State Hospital Consultation and advice to Senior Clinicians and Managers on structural and management issues within the service. For example advising on enhancements to relational security and therapeutic milieu to maintain safety and enhance patient well being and experience. Clinical supervision and training through ward based supervision for staff, clinical team supervision, clinical discussion groups for mental health professionals/staff (e.g. psychiatrists, nurses psychologists etc), specific training for Forensic Psychiatry trainees and Forensic Psychotherapists Strategic input into planning of services for patients with personality disorder presenting in forensic settings Contributing to high quality, evidence based provision of psychodynamic and mentalization based psychotherapy and the sharing of best practice Training: 1. Teaching and training other professionals and members of the multi disciplinary team in aspects of psychodynamic psychotherapy 2. INTRODUCTION This is an exciting time to be joining the State Hospital’s consultant medical staff. There is a national Forensic Mental Health Managed Care Network, which takes a pan -Scotland approach to the development of specialist services with a view to improving patient care and the flow of patients through the system (http://www.forensicnetwork.scot.nhs.uk/). The new State Hospital was opened in 2011. The hospital works closely with the various Forensic Psychiatry Services across Scotland and hosts the Forensic Network. The State Hospitals Board for Scotland is seeking to appoint a Consultant in Medical Psychotherapy to lead the development of high quality psychodynamic assessments and interventions for patients (male only) within the high security psychiatric hospital for Scotland and Northern Ireland. The post is for 5 sessions per week. The applicant will be a fully qualified (on the specialist register) Consultant Psychotherapist or be within six months of obtaining the relevant Certificate of Completion of Training. Candidates should be fully GMC registered and licensed medical practitioners. They must hold membership of the Royal College of Psychiatrists. They will have to evidence full engagement with appraisal and revalidation unless this is not applicable given their previous employment (i.e. having moved on from a training post). Direct clinical interventions for male forensic patients including individual and group psychotherapy, psychodynamic assessment contributing to multidisciplinary assessment and contribution to case based discussions and clinical management plans. b. Assist the service to deliver world class relational security and optimal therapeutic environments that manage risk and enhance emotional wellbeing c. To take a leading role in the planning, coordination and implementation, of a forensic psychotherapy service. This will include implementing an assessment service for people who require an in depth understanding of developmental issues, which should aim to improve their engagement and safe management. This may include recommendations about the therapeutic environment and also include specific work with staff that links with training, supervision and models of care and the provision of treatment. This service must form the capacity to evaluate itself and adapt to emerging evidence. d. To identify and co-opt existing resources and skilled individuals building a consensus between staff working in different disciplines. e. The post holder would be expected to take part in strategic discussions within the State Hospital and the Forensic Network about the future of psychological therapies to address personality disorders in the forensic sphere, and their role and relationship to different stakeholders (prisons, courts, criminal justice social work services etc.). f. To provide clinical expertise to support the work of multi-disciplinary teams, including contributing to case conferences, teaching seminars, clinical consultation and team supervision. g. To provide supervision and training to core and senior trainees appropriate to their level of experience, to contribute to the local forensic psychiatry training course, and to teaching and training to other professionals within the State Hospital and to the wider Forensic Network, via the School of Forensic Mental Health.. h. To collaborate with other professionals within the State Hospital to assure the provision and development of integrated referral pathways and patterns of care. i. To contribute to the strategic development of psychotherapy within the State Hospital and Forensic Network, including the Medical Advisory Committee. j. To participate in Clinical Governance programmes and furthering evidence based practice. k. The post holder will be expected to comply with the requirements under the Care Planning Approach and other State Hospital policies. l. To be involved in the teaching of junior doctors and medical students. m. The post holder must be CPD registered and maintain up to date knowledge and expertise. n. The postholder will participate in the consultant on-call rota, currently on a 1:12 basis. 2 CLINICAL GOVERNANCE, TEACHING AND RESEARCH The State Hospital is fully committed to evidence-based practice and the application of contemporary methods and standards of care and treatment. To this end, we have high expectations of clinical staff in the fields of clinical effectiveness, teaching and research. Full support will be available from The State Hospital's Clinical Effectiveness Department and from the Research Committee, which meets monthly to discuss and advise on research projects in the context of the hospital Research Strategy. There is a Clinical Governance Strategy and Implementation Plan, under the leadership of the Medical Director, and consultant staff are expected to participate in clinical governance activity. This is both to support the hospital’s clinical strategic aims and to assist consultants in building up evidence for their appraisal documentation. The State Hospital is recognised for General and Higher Professional Training. The postholder will be expected to supervise his/her specialist trainees in accordance with NHS Education Scotland and Royal College of Psychiatrists standards, and to participate fully in the local postgraduate teaching programme. This will include attendance at, and full participation in, Case Conferences, Journal Clubs and other training events. 3 CONTINUING PROFESSIONAL DEVELOPMENT (CPD) The State Hospital seeks to ensure that all staff have the opportunity to maintain and develop their clinical skills and will offer support to Consultant staff in maintaining their accreditation with the RCPsych through CPD. Consultant CPD peer groups are in existence in order to facilitate the development of Personal Development Plans to meet the requirements of the Royal College of Psychiatrists and the Consultant Appraisal system. It is recognised that training opportunities in other fields, including management skills, are important and the State Hospital will support these whenever possible. Individual development plans will be discussed annually as part of the Consultant Appraisal system and job planning. 4 MANAGEMENT The postholder will be expected to engage in such corporate management responsibilities as deemed necessary and appropriate, including providing psychiatric advice to selected working groups and project teams set up to implement the hospital’s Clinical Strategy, Local Delivery Plan and Clinical Governance Implementation Plan. There are also opportunities to take forward some of the necessary work in implementing and following through national standards of care not only in mental disorders but also in the physical health strategy for our patients. At the request of the Medical Director, bearing in mind clinical workload, the postholder will be asked occasionally to undertake Critical Incident Reviews in accordance with the hospital’s policy. 5 BACKGROUND The State Hospital is a national resource for Scotland and Northern Ireland providing psychiatric care in a secure environment. Committed to providing the highest quality treatment and care for patients while respecting their dignity and individuality, the State Hospital was the first NHS hospital in the U.K. to be awarded the quality standard, ISO9000 for its entire range of services. The State Hospital has an annual budget of approximately £35 M, employs around 766 staff, and has 140 beds. Specialised care is provided by multidisciplinary teams comprising medical, nursing, security, psychology, occupational therapy, social work and clinical pharmacy staff. The State Hospital has contracted with South Lanarkshire Council to provide its social work services. The social work team comprises a Team Manager, a Team Leader / Care Programme Manager and 8 social workers. There is a dietetics service and provision of primary care services in a Health Centre. There is a Physiotherapy Service. A well-staffed Psychological Therapies Service delivers individual and group-based protocol-driven psychological therapies to which patients are referred by clinical teams. An independent Patient Advocacy Service has been in place for some years and is seen as an important facility for patients, to which the hospital has committed more resources in the last financial year. The research and clinical effectiveness ethos is well established. In association with the University of Edinburgh, there is a Professor in Forensic Psychiatry (Honorary Consultant Forensic Psychiatrist) and a Lecturer in Nursing. There are also academic links with several Universities and Colleges. In an innovative development in adult learning for staff, the staff library has become part of the Learning Centre, with extensive IT equipment and staff on hand to assist learning activities. Supporting the delivery of care and treatment are the Maintenance, Finance and Human Resources Departments with a Support Services group fulfilling Information Technology, Hotel Services, Contracts and Purchasing requirements. There is an independently contracted Occupational Health and Safety Service, including health promotion advice, to which all members of staff have access, with if necessary access to a confidential counselling service. The Sports Hall facilities are available for staff to use at specific times, and there are other health promotion activities such as Fitech assessments, a weekly keep fit class and help with stopping smoking. Other medical staff The physical health of our patients is looked after by a contracted GP service, secondary medical care being obtained from NHS Lanarkshire. Management Structure The State Hospital is managed by the State Hospitals Board for Scotland, chaired by Mr Terry Currie. There are also 6 non-executive directors. The Medical Director is one of the executive directors to the Board and there is thus a close working link between the consultant group and the Board. The Medical Advisory Committee comprises all the medical staff of the hospital, including trainees, although only consultants contribute to the quorum and have voting rights. The Medical Director is responsible for liaising with the Medical Advisory Committee, although the MAC can communicate independently with the Senior Team.
