The Joliet Grade School Music Department will celebrate the end of another successful school year by having an outing at
Silver Cross Field for the Joliet Slammers vs. The Florence Freedom baseball game.
Please reserve your tickets immediately by completing and returning this form with your payment to your
child’s director. Please make checks payable to Joliet Grade School. (*all check numbers must be 500 or above)
You may order as many tickets as you like, as long as payment is received with the order. Orders will be filled
on a first-come, first-served basis.
Student Transportation to and from Silver Cross field must be provided by parents/guardians.
NOTE: Students must be accompanied by parents/guardians at the game. The directors will be present,
but supervision must be provided by parents. No tickets will be sold to 4 th and 5th grade students that do
not also purchase adult tickets. *Game begins at 7:05 P.M. Bus transportation will not be provided.

Music Department on Friday May 24, 2013. I will provide my child with transportation to and from the
stadium. I understand that students must be under the supervision of adults. I will either attend the game
myself, or I will send my child with another parent.
