This addendum #3
1.
Closing Time: In Section 2.1 of the Request for Proposals (as amended by Addendum #2)
under Closing Time, change the Date from “Tuesday, August 14, 2007” to “Monday, August
20, 2007”.
2.
Form of Proposal: In the Request for Proposals:
(a)
Delete Section 3.2(g) in its entirety (which includes the deletion of Schedule C-6 in
its entirety);
(b)
insert a new sentence at the end of Section 3.2 reading:
“In addition to the above, a Proposal should include:
(g)
a “Confirmation of Performance Security (as required by Section 3.6)”; and
(h)
Financial Statements as follows:
(i)
if the Proponent is a corporation, submit audited financial statements
for the past three (3) years (2004, 2005, and 2006), which may be in
the form of an annual report;
(ii)
if a Proponent is a private corporation and audited financial
statements are unavailable, then it should, as a minimum, provide a
letter from its auditor attesting to the accuracy of the financial
statements presented;
(iii)
if a Proponent is a large multi-divisional corporation and financial
information is reported on a divisional basis, then the Proponent
should submit financial information for the relevant waste
management division, in addition to the consolidated audited financial
statements.”
3.
Options: Delete Section 3.4 of the RFP entirely and substitute a new Section 3.4 as
follows:
“3.4
4.
Proponents may, at their discretion, in addition to or in substitution for a Proposal
contemplated by Section 3.2 of the RFP, submit an alternate proposal to perform
the Basic Services which is structured or will be performed in a manner or using
methods different to those as expressly or by implication described in the
Contract and the City may, at its discretion, evaluate such alternate Proposals
applying the evaluation criteria as set out in Section 4.3 of the RFP, and the City
may award the Contract on the basis of an alternate Proposal.”
Confirmation of Performance Security: Insert new Section 3.6 reading as follows:
“3.6
As confirmation of the availability of the Performance Security, include with a
Proposal either:
(a) if providing a performance bond, a completed “Agreement to Bond” generally
in the form attached as per “Schedule G – Agreement to Bond” to this RFP;
or
(b) if providing a letter of credit, a letter from the issuing financial institution
confirming that the letter of credit will be provided upon demand of the City
as a condition of award of the Contract.”
5.
Evaluation Criteria: Delete Section 4.3(d) of the Request for Proposals and substitute:
“4.3(d)
The Evaluation Team will consider the Proponent's response to “Schedule C-5:
Schedule of Quantities and Prices” and the information requested under
Subsections 3.2(g) and (h) of the RFP to consider the Proponent’s offer price(s)
for the performance of the Services.”
6.
Clarifications and Interviews: The City anticipates undertaking evaluation of the
proposals and may require clarifications and/or interviews from August 22 to August 31,
2007 and Proponents should be available during that time to provide responses as the City
may require.
7.
Schedule of Quantities and Prices: Delete Schedule C-5 in its entirety and substitute it
with the attached.
8.
Schedule A: In Schedule A, Section 4.8 Missed Collections; add sub-section 4.8.5 as
follows:
“4.8.5
Upon receipt of a complaint, whether from a resident directly or pursuant to
section 5.2.4 of this Schedule, the City will rely on electronic GPS vehicle tracking
data, as set out in Section 10 below, and/or field inspection, to determine whether
or not the complainant’s street had been serviced. If it is determined via the GPS
vehicle tracking data, and/or field inspection that:
(a) the street or complainant’s Collection Materials had been missed, the Missed
Collection processes under section 4.8 and/or resulting penalties under
section 17 will apply; or
(b) the street had been serviced, the City will assume that the resident or
complainant was late in placing their Collection Materials at Curbside, and
neither the Missed Collection processes under section 4.8 nor resulting
penalties under section 17 will apply.”
9.
Schedule A: In Schedule A, Section 8 Vehicles and Equipment; delete sub-section 8.1.3
and substitute it with the following:
“8.1.3

At the commencement date of the Term all vehicles to be used by the Contractor
for purposes of providing the Services shall be no older than eight (8) years. The
Contractor will be required to provide the City with a vehicle replacement
schedule no later than December 31, 2008 governing the subsequent years of the
contract. As of January 1, 2009, no vehicle used for the Services shall be older
than three (3) years.”
Potential Related Contracts: Insert the following as a new Section 4.12 of the RFP:
“4.12
The City has issued RFP’s for Yard Waste disposal (RFP 1220-30-40-07 Disposal of Residential Curbside Yard Waste Materials) and Recycling
processing (RFP 1220-30-41-07 - Processing & Marketing of Recyclable
Materials) and may, pursuant to those RFP’s, issue contracts and the Preferred
Proponent will cooperate with the City to settle the terms of the Contract that are
compatible with the contracts with third party contractors in the performance of
the Basic Services. The Yard Waste disposal site is operated by Fraser
Richmond Soil & Fiber Limited and located at No. 8 Road and Blundell Road,
Richmond, BC. The Recycling processing facility for a third party contractor
selected by the City will be located within the limits of the City of Surrey.”

Five Million ($5,000,000.00) Dollars for the full and due performance of the Services as described in
the RFP No. ____________ if the proposal of ________________ is accepted by the City
2.
It is a condition of this Agreement that if the above mentioned Proposal is accepted, the
Performance Bond must be submitted to the City prior to the commencement date of the Contract in
accordance with the RFP.

SCHEDULE OF QUANTITIES AND PRICES FOR 2008 CALENDAR YEAR SERVICES
Insert Annual Unit Price for each line item below.
The Estimated Number of Units is provided as reference in this regard, and as described in this Contract may not be the
actual number of units.
The following definitions will apply:
“Annual Unit Price” means the total price in Canadian Dollars to provide the Basic Services for that item for the entire
calendar year, and includes all costs of labour, equipment, materials, administration, scheduling, profit, overhead,
financing, and all other costs;
“COSF” means City of Surrey Facilities;
“LIPU” means the “Large Item Pick up Program” of up to 4 large household items per year per SFD;
“MURD” means Multiple Unit Residential Dwelling;
“SFD” means Single Family Dwelling;
“SS” means Secondary Suite
“Weekly Curbside Waste Collection” means the weekly collection, transport and delivery of Waste to the Waste
Facilities.
“Weekly Curbside Recyclables Collection” and “Weekly Recyclables Collection” means the weekly collection,
transport and delivery of Recyclables to the processing facility.
“Weekly Curbside Yard Waste Collection” means the weekly collection, transport and delivery of Yard Waste to the
Yard Waste Facilities.
