Please use the scroll bar to your right to read through all instructions and then
fill out the questionnaire. This form requires you to tab from each field in
which you insert information. When you are finished and have saved the
document, e-mail it as an attachment to:dlattimr@Memphis.edu, or fax to Dan
Lattimore at 901-678-4049. You can also mail it to Dan Lattimore, Dean,

This form enables you to rate both the instructor and the course on several characteristics. Please
respond as accurately and honestly as you can. The “free response” section at the end includes
three open-ended questions for your comments.
Use the scroll bar on the right of your screen to move down through these instructions. The
highlight is on the first answer box of question #1. Just press the tab key to move to each
successive box and type an X on your choice of the available answers.
You can tab all the way through the survey and cycle back through to change any answers.
When you are finished, click on <file/save as> and save this document with the word SETE,
followed by the course number, semester, and year. For example: SETE7100F99.
All responses will be tabulated anonymously to insure student confidentiality. The feedback
reports on SETE scores are not given to instructors until the semester following the one
evaluated.
