This form is to be completed for every expedition (practise & qualifying) that is undertaken for the Duke of Edinburgh’s Award that is NOT in WILD COUNTRY. 2. If going into Wild Country, you need to complete a GREEN FORM, and send a copy of this instead. 3. Don’t forget to send a NAN (Night’s Away Notification) to the appropriate people. 4. It must be accompanied by a route card or photocopy/tracing of the route. 5. All paperwork will be stored for future reference under the rules of the ScoutsWales DofE Award Operating Licence. 6. The Expedition Request Form MUST be sent or emailed at least 4 weeks in advance of the venture to the Area DofE Co-ordinator.
