CONTRACT MANAGEMENT Document 6: Hazard Disclosure Form Last Update: 30 September 2009 Owner: Manager OHS The University has a duty to disclose matters that may be relevant to safe completion of the contract. This document when completed is to be kept with the current version of the Contractor’s Health and Safety Plan and forms part of the contract documentation. Further guidance regarding the selection of the most suitable control methods is provided in the Guidelines for Job Safety Assessments. In summary the selection of suitable control measures should take into consideration: Level of risk Hierarchy of controls Practicability of implementation Hazard Identification and Control Table The following table provides examples of possible hazards that should be drawn to the attention of the contractor before the commencement of works.. These examples are provided as a guide only and important site specific factors must also be considered. Note also that this table of examples does not include all possible hazards. 1. Other contractors Details Vehicles moving onto or through the site Pedestrians moving through or onto the site The likely presence of public at or near the site Adjacent works that may have an effect on the safety of the site Unusual or unexpected fire hazards such as wall lining and insulation The known presence of electrical wiring and lines Unprotected electrical outlets Noise sources High pressure pipes and lines Storage or presence of chemicals on site Fumes or vapours from adjacent sites The presence of contaminated soil The presence of unsafe infrastructure such as ladders, stairs etc. Equipment on the site that is unsafe to be used The presence of radiation sources Presence of asbestos The presence of biological materials that could cause infection or ill health The location of untreated cooling towers or water storage Confined spaces or other hazardous areas Hazards peculiar to the site The likely presence of other contractors or parties on the site Disclosure of OHS Hazards The University is aware of the following unusual or unexpected OHS hazards/issues that may affect the contractor or their employees. This disclosure is made in good faith and does not imply any acceptance of liability. This disclosure does not remove or diminish the contractor's responsibility to assess the proposed works and implement appropriate control measures. In particular the presence of any other parties on site works must be noted and an appropriate coordination is the responsibility of the contractor.
