Yorkhill Division Patient Group Direction 7.2 THE ADMINISTRATION OF ORAL IBUPROFEN (P) RHSC PATIENT GROUP DIRECTION 7.2 PATIENT GROUP DIRECTION FOR THE ADMINISTRATION OF ORAL IBUPROFEN BY NURSES WITHIN THE ACCIDENT AND EMERGENCY UNIT AND SHORT STAY WARD PLEASE NOTE: This document authorises the administration of IBUPROFEN by the identified group of nursing staff to patients who meet the criteria for inclusion under the terms of the document. THE NURSE SEEKING TO ADMINISTER IBUPROFEN MUST ENSURE THAT ALL PATIENTS HAVE BEEN SCREENED AND MEET THE INCLUSION CRITERIA BEFORE ADMINISTRATION TAKES PLACE. Professional Responsibility The registered nurse must adhere to the following: NMC Code of Professional Conduct (NMC 2002), NMC Guidelines for the Administration of Medicines (NMC 2004), NMC Guidelines for Records and Record Keeping (NMC2002), Yorkhill Division Waste Disposal Policy (2004), A Report on the Supply and Administration of Medicines under Group Protocol (Crown Report) (1999) (DOH). PATIENT GROUP DIRECTION FOR THE ADMINISTRATION OF ORAL IBUPROFEN Clinical condition or situation to which the PGD applies Definition of situation / condition: Clinical criteria for inclusion: A child with a temperature above 38C and/or relief of mild to moderate pain Children from three months of age with a temperature of 38C or above and/or relief of mild to moderate pain Paracetamol has not been successful in reducing temperature and/or relieving pain Child has received paracetamol for weight within the four hour period or has had the maximum dose within 24 hours Criteria for exclusion: Renal, cardiac or hepatic impairment History of asthma Hypovolaemia/significant dehydration A known allergy to Ibuprofen History of peptic ulcer Children under three months of age or less than 5 Kg in weight Action if excluded from treatment: Children who are excluded or do not wish to receive Ibuprofen will be assessed by a doctor. CAUTION Possible Adverse Drug Reactions (ADR) Side effects are uncommon, but the most common reactions are gastrointestinal i.e. nausea, vomiting, diarrhoea, abdominal pain, melaena and haematemesis. Hypersensitivity and skin rash may also occur. Any adverse reaction should be reported immediately to medical staff and a yellow card completed if serious ADR occurs Characteristics of staff authorised to take responsibility for the supply or administration of medicines under the PGD Qualifications required: Additional requirements, Specialist qualifications, training, experience and competence necessary: Continuing Training requirements: Registered Nurse with current NMC registration. Grade D must have two years post registration and E grade and above must have 6 months experience in A&E and SSW Received theory and supervised documented practice by one of the named Independent Nurse Prescriber identified within this PGD. Be deemed competent to transcribe and administer Ibuprofen by the Independent Nurse Prescriber Once deemed competent the Nurse will be added to the list of authorised users which is attached to this PGD and a copy is available in pharmacy Demonstrate capabilities with regard to safe and competent practice and agreeing to continuous assessment in transcribing and administration of Ibuprofen. Page 3 RHSC PATIENT GROUP DIRECTION 7.2 Description of treatment available under the PGD Name of medicine: Ibuprofen 100mgs per 5ml Legal status (POM/P/GSL): P Dose/s Route/Method of Administration: 7.5mgs per Kg in weight Maximum dose 400mg Oral Frequency of dose: Single dose Referral for medical advice The child’s temperature and/or relief of pain should be reassessed after one hour by nursing staff. Medical staff should be notified if no response is noted and no further medication i.e. paracetamol or lbuprofen can be given. Additional Information The need for Ibuprofen should be assessed by measuring and recording the child’s temperature and/or assessing pain. Any current medication the child is taking should be recorded on the accident &emergency card/case history sheet. The child’s age and weight should be obtained. The registered nurse should decide an appropriate dose and route of Ibuprofen using the dosage guidelines. The Ibuprofen should then be transcribed, prepared, checked and administered as per “Yorkhill NHS Division Nursing and Midwifery Medicine Policy”. Details of treatment records required: The date, time, dose and route of Ibuprofen should be recorded in the accident & emergency/drug kardex and signed by both the transcribing nurse administering the drug and the checking nurse.
