Please complete the form below to indicate your interest in becoming a State Coach of the MLD
program in Ohio. To be considered for a coaching position, you must be a League member and agree
to participate in the activities outlined on page 2. The DEADLINE to submit this form for

I understand that as a coach, I agree to:
o Attend MLD Coaches’ annual training, to next be held April 17-18, 2015, in Columbus, OH.
Note: LWV Ohio will pay for the materials, hotel (two people per room) and meal expenses
associated with the coaches’ training.
o Communicate monthly with the MLD Teams of my assigned local Leagues to help them develop
and successfully implement an MLD program specifically designed for their League.
o Participate in a monthly phone call (45-60 minutes) with Ohio’s Shur Fellow (LWVUS liaison to
the MLD program) and the Ohio MLD Coaching Team.
o Participate in a brief on-line survey each month from LWVUS.
o Regularly communicate with the Ohio MLD Coordinator concerning the progress of my assigned
local League(s).
