Telephone : 0844 477 8548 24 Hours Fax : 01926 427254 January 2010 APPOINTMENTS The Surgery is open each weekday as follows; Monday and Friday 8.30 – 6.30 Tue, Wed and Thur 8.30 – 12.30 1.30 – 6.30 You may contact the surgery by telephone 8.00-12.30 and 1.30-6.30 All consultations are by APPOINTMENT ONLY, which can be made either by telephone, or in person at reception. Our appointments system is designed to enable fast access to see your doctor. Half the number of appointments available can be booked in advance, and the remainder will be released on the day. To book ‘on the day’ appointments please telephone the surgery after 8.00 am. Please note a computer sets the telephone system and you will not be put through until after 8:00am. If you telephone and the appointments have already been allocated you will be asked to phone again on the next day. Alternatively you may book an appointment in advance.URGENT medical appointments will always be available on the same day. Please remember, if you are unable to keep an appointment, let us know as soon as possible, so that we can use it for someone else. EXTENDED HOURS APPOINTMENTS These are pre-booked appointments available in the evenings, that are available to all but are especially to help patients who find it difficult to attend during regular hours. REPEAT PRESCRIPTIONS This system is for ROUTINE drugs that have been authorised by your Doctor and are held on our computer. If your prescriptions are on the computer and have been authorised by your Doctor, then they will be printed on the right hand side of your prescription as a ‘tear off slip’. Please use this where possible to re-order your prescriptions. PHONE CALLS TO YOUR DOCTOR If you wish to speak to your doctor, or a nurse, please be ready to leave your name and telephone number and the doctor or nurse will ring you back, usually at the end of surgery. Tick the drugs that you require and either; □ Post to us (write ‘Prescription’ on the envelope). □ Hand in at reception. □ Fax us on 01926 427254 HOME VISITS Please, only request a home visit if this is essential. Where possible please telephone before 10.30 am, giving some indication of the problem so that the doctor can plan his visit. Requests for urgent visits are dealt with at the time of the request. Whenever possible we ask you to come to the surgery, not only because home visits are time consuming, but also because we have better facilities for examining and treating you in the surgery. HOW TO COLLECT □ Collect from the surgery allowing 2 working days from request □ Enclose an SAE and we will post it to you □ Several local chemists will collect, and then deliver medicines. Check with your chemist and once agreed then please write the Chemist’s name on the order form and ‘Deliver’. If you wish to request drugs that are NOT on your re-order form you should make an appointment to see the Doctor for the prescription. If you are a new patient to the practice please ensure you have discussed your repeat prescription requirements with your Doctor so that they can be entered on to our computer system, before you need to order them. HOW TO ORDER As these are regular drugs, please request in plenty of time. We ask you to allow 2 working days following a request. THE PRACTICE POLICY IS NOT TO TAKE ANY PRESCRIPTIONS OVER THE TELEPHONE – THIS CAN LEAD TO ERRORS PLEASE USE THE TEAR-OFF SLIP ATTACHED TO YOUR PRESCRIPTION INSURANCE REPORTS AND SPECIAL EXAMINATIONS Medical examinations for insurance or other purposes eg HGV licence, hazardous sports, require special appointments for which you should contact reception. The receptionist can advise you of the fee that such examinations usually require. Reports and forms for many purposes, eg private health insurance, holiday cancellation, etc, are completed by the doctors outside of consultation time. The receptionists can advise you of the fees payable. These are based on BMA recommended rates to reflect both the responsibility involved and the work for both doctors and staff. IF YOU HAVE A DISABILITY The surgery has a ramp making it accessible to those who use wheelchairs or walking aids. Dr Bonsall and Dr Foster usually work upstairs. They can see you downstairs if needed, but it may be helpful for you to mention your special needs when making your appointment. If mobility problems prevent you getting to the surgery, doctors or nurses can visit you at home. If you have a hearing impairment please ask for a portable hearing loop at reception. PROTECTING YOUR INFORMATION Everyone working in the NHS has a legal duty to maintain the highest level of confidentiality about patient information. All staff are asked to sign a Code of Conduct on confidentiality. We need information about yourself so that we can provide you with the best possible care and treatment. We keep this information, together with details about your care, in your medical record, to ensure that your doctor or nurse has accurate and up to date information. We only pass this information to other healthcare professionals who have an interest in your health and well being. Sometimes we are asked to provide information to solicitors and insurance companies. We only do this with your written permission. Increasingly we are asked to participate in audits and research on behalf of the NHS and wherever possible we remove details that identify you as an individual. In certain circumstances we are obliged by law to report information to appropriate authorities. EMERGENCIES OUT OF HOURS The surgery is closed after 6.30 each evening, at weekends and on Bank Holidays. If you require EMERGENCY medical care when the surgery is closed telephone the surgery number – 0844 477 8548 and follow the directions given. Alternatively, telephone direct to the Out of Hours Service 0300 130 30 40 This service is not designed to deal with work that could wait until normal working hours. Therefore we ask you to call the Out of Hours Service ONLY when this is really necessary, in other words if the symptoms are worrying and if you think harm would result by waiting until the surgery is open. Calls to the 0844 number for the surgery cost a flat rate of 3.9p per minute + a 2.9p call set up fee. This is the same charge as the BT Standard Rate tariff calling from a BT Landline. Calls from mobile phones to 0844 numbers are subject to the providers’ charges. All 08 (non-geographic numbers) tend to be excluded from calling plans. If you have any comments, complaints or compliments about any aspect of the service please contact our Practice Manager. Complaints will be dealt with according to our established procedure; a copy is available at the reception. PRACTICE NURSES We have three Practice Nurses and consultations are on an appointment basis. The nurses are available for health and lifestyle advice, and carry out a wide range of routine nursing procedures. They also manage chronic illnesses – Diabetes, Asthma, COPD, High Blood Pressure and Heart Disease, and female health issues – Contraception, Smears, Menopause and HRT. RECEPTIONISTS Please remember that when the receptionist asks you questions, they are carrying out our policy, which is aimed at helping the practice to run smoothly. They have to judge the urgency of your request, so try to give them the necessary information. They are bound by the same rules of confidentiality as the doctors and nurses. NURSING CARE AT HOME Our attached Community Nurses offer general nursing services at home for those who are unable to visit the surgery. The doctors, practice nurses, or hospitals can refer you for this. MENTAL HEALTH PROBLEMS Our Community Psychiatric Nurse offers advice and help for mental health problems, working closely with a varied group of professionals at St Mary’s Lodge/Yew Tree House. Attendance is normally through a doctor’s referral. COMMUNITY MIDWIFE Our midwives hold a weekly antenatal clinic in the surgery, on Friday afternoons. HEALTH VISITORS The health visitors are registered general nurses with special training in infant and child-care and development. We recommend children’s health checks at different stages to assess your child’s progress and development, as well as immunisations against infectious diseases. The Health Visitors are based at Crown Way Clinic Tel: 01926 332242 and Lillington Children’s Centre Tel: 01926 337506 CHILD HEALTH CHECKS These are held weekly on Friday afternoons with Dr Foster. Invitations are sent by the Health Authority and are by appointment only. MINOR SURGERY The doctors are able to carry out minor surgery such as removal of cysts, skin lumps and warts. BLOOD TEST APPOINTMENTS Most blood tests are taken in the surgery by a phlebotomist. The doctor or nurse will issue you with a form and an appointment should be made either by telephone or calling at reception. Doctors may recommend blood tests at home for housebound patients. NEW PATIENTS All new patients are offered an appointment for a simple health check. This enables us to obtain details about your past medical history and to provide improved care.
