Scholarship Request Form – Fall 2015 Tackle Football Season
Please fill out the form below to request a scholarship for the CYFA fall tackle football season.
Please note that the scholarship may be for partial or full registration costs. Please submit this
form via email to: lee.kraus@utsouthwestern.edu by the June 25, 2015 deadline.
(The CYFA board will review all applications and notification will be given in time to complete registration by the
registration deadline.)

Scholarships awarded are based on annual budget and must be approved by the CYFA Board of Directors. Prior
year tax returns or other financial proof may be requested. One scholarship allowed per family. Equipment is the
responsibility of the player.
Parent/guardian may be requested by the CYFA to volunteer during the season to help with scorekeeping, clock,
or other game management or event assistance.
Player must be dedicated to attend practices and games and be a positive team player. If there is an emergency or
other conflict that results in missing a game, the player must let the coach know well in advance.
