Saint Ann Church Registration Form Instruction Sheet School Year 2011-2012 PLEASE NOTE: Tuition ONLY is waived for Catechists and Co-Catechists – fees must be paid 1. Family Name – some children have different last names – please enter the last name of parents as registered in the Parish Census Listing 2. Family E-Mail – please enter the preferred e-mail for the Office of Religious Education and/or catechists as we use e-mail for contact re: class updates, information, changes, etc. ONLY. 3. Home Telephone – please include area code. 4. Address – please complete 5. Preferred Emergency Phone – please include area code and indicate which parent. This will be the FIRST call we make, so please be sure about the number given 6. Alternate Emergency Phone –number we call if unable to reach the above number. 7. Volunteers Section – PLEASE NOTE: where the VIRTUS program (Protecting God’s Children) is required, it must be completed BEFORE beginning that ministry. Call the ORE for more information. NOTE: Once you have completed this program, you are compliant for any ministry in the Church. Indicate which parent is volunteering for each ministry. 8. Names of children – please list name, the grade entering in the fall and tuition amount, also sacramental fee (grades 2 and 10) if applicable and calculate your check amount. We will double-check these entries and refund you any overpayments. (If catechist or co-catechist, tuition only is waived – do not enter any tuition monies here, but do include any fees including Grade 10 – The Youth Spectacular fee of $50 must be paid.) 9. Grade 9 and 10 - tuition is payable in grade 9, while the Confirmation fee - $60 (gown rental, retreat costs, books) and Youth Spectacular Fee - $50 are paid in grade 10. Registrations must be received by September, 1, 2011 because Saint Ann Program activities begin early in September and attendance is expected. (See the back of Registration Form). 10. Special Needs Information – as noted on form. 11. Class Assignment – please note that no student (K-10) will be assigned to classes until registration and payment is received in the Office of Religious Education. Any registration received after June 30, 2011 must include an additional $20 fee. Due to the disruption caused by late registrations, none will be received on any day of classes once they begin in September. Registrations will be accepted in the ORE only. Please do not hold up your child/ren’s registration because of financial issues. Since we only have 14 classes, missing any will impact your child. Please contact the ORE as confidential arrangements can be made. (See Registration Dates) 12. Class Sizes – we are trying to keep K -6 class sizes at 14-16 students, and grades 7 and 8 at 10-12. Since classes are formed on a first come-first assigned basis, once we exceed those numbers we will be looking for additional catechists 13. Home Schooled Students – Please call the Office of Religious Education for information. 14. NEW STUDENTS must provide Baptismal and sacramental records unless child was baptized here at The Church of Saint Ann. If so, please give us the baptismal year and we will research it. 15. REVERSE SIDE - Grades 9 and 10 Stewardship – Parents are expected to volunteer for service in one or more areas of Faith Formation. The events listed are for parents. (Students will be participating) and please name the parent volunteering. REMINDER: please be sure to complete the permission slip for Grade 10 students for Youth Spectacular attendance and read the Code of Behavior. The event is Sunday, 9/18/2011. And thank you! 16.
