Social Services Information System (SSIS) 90 Day Transition Plan The 90 Day Transition Plan is developed in addition to the Out-of-Home Placement Plan (OHPP) to address the needs of every youth expected to ‘age out’ of foster care. It is ideally completed 180 days prior to the last anticipated date the youth will be in care, but is required to be completed within 90 days of the youth’s last day in placement. It may also be used to address issues specific to 18 to 21 year old youth remaining in – or returning to placement under Extended Foster Care. Service Plans Folder New State Service Plan Document Template 90 Day Transition Plan To open the 90 Day Transition Plan: 1. Right-click on the Service Plans folder and select New State Service Plan. 2. Select 90-Day Transition Plan from the Document template drop-down menu. 3. The Setup tab of the 90 Day Transition Plan displays. To complete the Setup Tab: 1. Click anywhere on the Setup tab screen to enable data entry. 2. Select yes or no to display the Language block. 3. Enter a date in the Effective from date field. 4. Enter a date in the Effective to date field. 5. Enter the name of the identified youth in the Participant field. Setup Tab – Insurance Field 6. If the youth has private insurance coverage (which merges from the Health/Insurance folder) select it beneath the Insurance section. 7. Ensure that the primary caseworker’s name is reflected in the Staff field. 8. Click Save. 9. Click on the Document tab. Review the information on the Title Page screen for accuracy, and answer the three questions in the corresponding Editor boxes. Click Complete. 2. The Statutes screen lists three links to the MN Revisor of Statutes website. The statutes listed define three requirements for information to be provided to the youth prior to discharge from placement. Click on any link to review these requirements. Click Complete to print this page for the youth. 3. Answer the questions related to Housing, Education and Employment in the corresponding Editor boxes. Click Complete. Health Information Screen – Appointments Grid 4. On the Health Information screen, review any private health insurance information selected from the Setup tab as applicable. Answer the question regarding health care providers, and enter the youth’s current health care Providers, Addresses, Appointment Types, and the Date, Time, and Phone in the corresponding grid. Click Complete. 5. Answer the questions on the Support Services screen, and in the grid, enter the Name, Relationship, Home Telephone, Cell Phone, and Email Address of all support persons to the youth. Click Complete. Select the items listed on the Vital Documents screen which have been provided to the youth. Note the link referencing this statutory requirement. Click Complete. 7. Crisis resources for the youth are entered on the Contacts screen. Enter the Type of Contact, the Name, Address, and Phone/Email. Click Complete. Contacts Screen – Free Credit Reports Phone and Link 8. Note at the bottom of the Contacts screen that the telephone number and website display to obtain a Free annual consumer credit report. 9. The name of the Youth and Social Worker auto-fill on the Signature Setup screen. Click Complete. 10. Click Save. Hint: In order for text to print on the service plan, the Complete box at the bottom of each screen must be checked. Once the Complete box is checked, a red checkmark displays in the Navigation bar. Hint: Right-click on the Document tab, or click on the 90 Day Transition Plan in the Tree View, to print the plan. Activate the service plan once signatures are obtained.
