Child/Adolescent and Adult Activity & Nutrition Tool Kits for
Healthcare Providers Available
Despite the well-known health risks and economic impact associated with
obesity, rates in Missouri adults continue to rise. Health care
professionals, through frequent contact with the public, have a unique
opportunity to assist in changing nutrition and physical activity habits to
promote a healthy weight. In addition, when health care providers talk,
patients and the public are more likely to listen. The Health Care Work
Group from the Missouri Council for Activity and Nutrition (MoCAN) has
developed a tool kit for health care providers to incorporate the latest
screening and treatment recommendations into their daily practice.
The Activity & Nutrition Tool Kit includes the following resources:
 Body Mass Index Screening Table
 Pocket Guide: “Assessment and Treatment of Adult Overweight and Obesity”
 Patient Readiness Checklist
 Communication Guidelines to Promote Behavior Change
 State of Missouri Consensus Screening Guidelines for Pre-diabetes and Diabetes
 Patient Handouts on nutrition and physical activity tips and goal setting
This tool kit was developed, with extensive input from a collaborative team of Missouri’s health
care leaders, to support the promotion of healthier weight with patients. The resources
summarize recommendations from scientific literature and expert work groups on obesity. The
tool kit is available to download from the Missouri Department of Health and Senior Services
Web site at http://health.mo.gov/living/healthcondiseases/obesity/adulttoolkit.php. Free copies
can be ordered by printing an order form (stock #146) from
http://health.mo.gov/warehouse/580-039.pdf and faxing it to the MO DHSS warehouse at

The Health Care Work Group from the Missouri Council for

health care providers screen, assess and treat weight issues in
youth. The tool kit contains:

Adolescent Overweight”
 Patient Handouts
 Assessment and Counseling Form for Pediatric Overweight
 BMI Wheel
 BMI-for-Age Percentiles Growth Charts
 Blood Pressure, Diabetes and Cholesterol Screening Charts
These resources summarize recommendations from scientific
literature and expert work groups relating to child and adolescent obesity, including the recently
released Expert Committee Recommendations on Assessment, Prevention, and Treatment of
Child and Adolescent Overweight and Obesity. The tool kit is available to download at
