Applications are invited for this replacement post in the context of a colleague’s retiral, to join a
cohesive, established team of paediatricians in delivering children’s services in Ayrshire. This a
community paediatric post and would be based in the department of Child Health in Rainbow House
Child Development centre in Irvine.
We are looking for an enthusiastic speciality doctor community paediatric colleague to join a team of 5
Speciality Doctors and 4 Consultant Paediatricians covering the three localities of Ayrshire. The
successful applicant would provide Specialist Community clinics in their locality. In addition, the
successful candidate will be supported in developing an interest in adoption and fostering and
vulnerable children. Currently adoption medicals, reports and panel attendances are managed by a
speciality doctor in each locality of Ayrshire, with a contribution to Child Development work, as well as
one speciality doctor per locality. This post looks to replace that resource within South Ayrshire.
Services for Looked After and Accomodated Children (LAAC) services are nurse led. There is a
Consultant Lead for Looked After and Accomodated/Vulnerable Children and it is anticipated that the
successful applicant would work closely with this colleague. There are, in addition Consultant leads for
Neurodisability and Child Protection based within rainbow House
Community Paediatrics is integrated in to Children’s Services. There will be opportunity to provide input
to general paediatrics if so desired.
Applicants will be appropriately experienced with Full GMC Registration with a current Licence to
Practice, and will have completed 4 years full-time postgraduate training, 2 years of which shall be
within relevant specialties to Paediatrics. Membership of the Royal College of Paediatrics and Child
Health (MRCPCH) or equivalent is desirable.
The Terms and Conditions of service are those determined by the New Terms and Conditions of the
Specialty Doctor grade and is offered on a 8 Programmed Activity basis. Applications from individuals

JOB DESCRIPTION
SPECIALTY DOCTOR - COMMUNITY PAEDIATRICS WITH AN INTEREST IN ADOPTION AND
FOSTERING/VULNERABLE CHILDREN
1.
INTRODUCTION
This replacement post (8 PAs) is intended to support delivery of Community Paediatric Services in Ayrshire. It is
envisaged that the appointee will develop and subsequently provide input to the local adoption and fostering
panel as the Paediatric Medical Advisor, in addition to maintaining a developmental caseload. The appointee will
work closely with the established Community Paediatric Team and will be supported in developing their role by
the Consultant Lead in Adoption and Fostering/Vulnerable Children and by the locality based Consultant in
relation to developmental assessment and liaison work.
There is at present no on call commitment in the post however there is an understanding that this could be
reviewed if necessary as a result of service reconfiguration in the future.
NHS Ayrshire and Arran has a total population of approximately 380,000 with 72,000 children under the age of
16 years.
2.
FACILITIES, RESOURCES AND ACTIVITY
2.1 FACILITIES
Community Paediatrics and Community Child Health:
The Child Health Service is delivered from numerous locations across Ayrshire and Arran. The Child
Development Centre (Rainbow House) and main Child Health Offices are located at Ayrshire Central
Hospital in Irvine. Rainbow House was recently refurbished and extended to provide a state of the art
facility. The Community Paediatric Team, comprising Medical, Nursing, Physiotherapy, Occupational
Therapy, Clinical Child Psychology, Speech and Language Therapy and administrative staff, is located
within Rainbow House. Much of the clinical work of the Team is delivered at locations outwith the Unit.
In addition to neurodevelopmental clinics, specialist clinics held in Rainbow House include, Epilepsy,
Neuromuscular (principally Duchenne MD), Communication Disorders(ASD), Tone Management and
Botulinum Toxin. Each locality is served by two paediatric speciality doctors to provide support for
nurseries and schools and liaison with Education and Social Work. Special interests include audiology
and hearing impairment and autism assessment. In each area one of the speciality doctors provides
input to adoption.
General Paediatrics:
The Paediatric Unit at University Hospital Crosshouse is responsible for the in-patient and out-patient
care of the section of the population between birth and the 16th birthday. A major reconfiguration of the
service took place in 2006, bringing all inpatient services on to one site. The general paediatric unit at
University Hospital Crosshouse was extended and completely refurbished to provide co-location of a
dedicated Outpatient Suite with 6 consulting rooms, a 10-bedded Children’s Assessment Unit and a
dedicated inpatient unit including not only medical paediatrics but also designated paediatric ID and
dermatology beds and paediatric surgical specialities (general paediatric surgery, orthopaedics, ENT,
dental surgery, plastic surgery and ophthalmology). There are 23 (26 during the winter months) inpatient
beds, including 4 in a dedicated adolescent facility. The majority of patients for elective surgery in the
above specialities are catered for in the Day Surgery Unit (separate from the inpatient facilities).
The University Hospital Ayr also has a dedicated Paediatric out-patient suite. In addition, General
Paediatric out-patient clinics are held at Ayrshire Central Hospital in Irvine, at East Ayrshire Community
Hospital in Cumnock, at the Davidson Hospital in Girvan, at the Brooksby Centre in Largs and at the War
Memorial Hospital in Lamlash on the Isle of Arran. Most clinics are of a general nature but there are
dedicated clinics for children with cystic fibrosis and diabetes. In addition there are joint clinics in
cardiology, endocrinology, epilepsy, gastroenterology and genetics held locally with visiting specialists
from Yorkhill.
Neonatology:
Ayrshire Maternity Unit is the only maternity unit in Ayrshire, catering for approximately 3,800 deliveries
per annum. There is a 20 cot Neonatal Unit with 5 designated intensive care cots and a 4-bedded
transitional care area in the postnatal ward. The Neonatal Unit provides level 3 neonatal intensive care
and has facilities to provide conventional and trigger ventilation, high frequency oscillation and nitric
oxide management of neonatal respiratory problems. There is an ultrasound scanner with colour Doppler
and a telemedicine link to Consultant Cardiologists and Paediatric Surgeons at Yorkhill. There are no
facilities for neonatal surgery locally. There are currently five Advanced Neonatal Nurse Practitioners in
post.
Education:
Undergraduates from Glasgow and Dundee Universities come to Ayrshire for paediatric blocks.
Foundation Year 2 and General Practice Specialty Trainees in their 2nd year of training come for 4 month
blocks of paediatric training. Specialty Trainees on the West of Scotland paediatric rotation come to
Ayrshire at various stages of their training for periods of up to one year on our middle grade rota. Both
Paediatric and Community Child Health staff attend joint CME meetings on a monthly basis. There are
also a number of regular Journal Clubs.

3. FUTURE PLANS FOR THE DIRECTORATE
Various challenges face our service at present. We anticipate a steady reduction in middle grade posts over the
next few years and are planning to cope with this using a combination of strategies including development of
more nurse practitioners and specialist nurses as well as changes in consultant working patterns.
We also face challenges with future proofing a service to cope in the longer term with retirements and the
ongoing reorganisation of children’s services. We would anticipate further amalgamation of traditionally
community based and acute services in terms of providing a single well coordinated service to meet all the
health care needs of the children of Ayrshire.
4. PROPOSED WEEKLY PROGRAMME
The appointee will be fully supported by the Clinical Director, and the Clinical Leads for Looked After and
Accomodated/Vulnerable Children and the Health Care Manager in providing services for looked after and
accommodated children. This will involve close work with colleagues already working within the service as well
as support to promote developments against national standards.
A prospective timetable is offered; while total sessions are correct, the times of different activities may change
depending on availability of clinic space etc.
The job plan will be reviewed with the successful candidate no later than 3 months following appointment and
where possible discussion may take place in advance of appointment. Job plan review thereafter will be no less
frequent than annually.
The agreed job plan will include all the specialty doctor’s professional duties and commitments, including agreed
Supporting Professional Activity (SPA).

Notes on the Programme
Patient Administration. This activity covers the management of individual patients including Out Patient
administration, results reporting, letters/’phone calls to parents, carers, GPs and members of the wider
multidisciplinary team involved in the patients’ care.
Travel: Any travel allocation will be included within the Total Programmed Activities and will be determined by
location at which Direct Clinical Care and Supporting Professional Activities are carried out.
Supporting Professional Activities: A minimum of 1 SPA is included in the indicative job plan, amounting to
168 hours per annum which shall normally be sufficient to reflect activities such as revalidation, appraisal,
personal audit, and professional development (occurring outwith the 30 days of study leave entitlement in any
three year period). Time permitting, it may also cover minimal teaching, training and non-clinical administration.
The exact timing and location of SPAs, and flexibility around these, will be agreed during the 1:1 meeting with
the Clinical Director/Associate Medical Director and included in the prospective job plan.
On call arrangements: There is at present no on call requirement in the post however it will be accepted that
this may become necessary due to service reconfiguration and this would addressed at that time.
5. THE POST: SPECIALTY DOCTOR COMMUNITY PAEDIATRICS - ADOPTION AND FOSTERING
The postholder will be accountable to the Clinical Director who will agree the Job Plan.
He/she will be expected to work with local managers and professional colleagues in the efficient running of
services and will share with Consultant colleagues in providing the medical contribution to management.
Subject to the provisions of the Terms and Conditions of Service, he/she is expected to observe the Division’s
agreed policies and procedures, drawn up in consultation with the profession on clinical matters, and to follow
the standing orders and financial instructions of NHS Ayrshire & Arran.
The postholder is required to comply with the Organisation's Health and Safety Policies.
5.1
Resources
The staff resources of the Directorate are listed elsewhere. The postholder will have access to such general
administrative support as is required for the discharge of his/her duties and responsibilities.
This will include the provision of adequate secretarial and clerical support and the availability of accommodation,
equipment etc.
The postholder will receive support from such other professional staff as are employed within the Division and
are deployed to his/her area of patient care.

Duties and Responsibilities
The main duties and responsibilities of the post include:



The new appointee will be responsible, to consultant colleagues, for the care and management of
children in Ayrshire referred for developmental assessments, liaison with education and social work and
as medical advisor to adoption and fostering panel in South Ayrshire.
Responsibilities for carrying out teaching, accreditation and examination duties as required, and for

Annual Appraisal & Job Planning

The postholder will also be required to participate in annual appraisal. Job planning is linked closely with, but is
separate from, the agreed appraisal scheme for consultants. The job plan review will take into account the
outcome of the appraisal discussion and reflect the agreed personal development plan.

amended from time to time.
This post involves exposure-prone procedures and so you will be required to undergo Enhanced
Immunisation clearance checks. You will also need to be cleared by Disclosure Scotland PVG.

SUPERANNUATION
You will be eligible to participate in the Scottish NHS Scheme. Superannuation benefits
accrued in the NHS Scheme elsewhere in the UK can be directly transferred to the Scottish
Scheme by arrangement between the two pensions agencies. The transferability of other
public sector pension scheme entitlements may be possible and may be explored on
appointment.

CAR LEASING
A car lease may be available in accordance with NHS Ayrshire and Arran’s Car Leasing
Policy.
6.
LOCATION
For employment purposes, the base will be either University Hospital Crosshouse,
University Hospital Ayr, Ayrshire Central Hospital, or Ailsa Hospital. The post may require
you to travel to and work at various locations in Ayrshire and Arran and occasionally further
afield. Home to work expenses will be met by yourself. Travel expenses during the
course of work will be met by NHS Ayrshire and Arran.
7.
TRAVEL EXPENSES INCURRED WHEN ATTENDING FOR INTERVIEW
Reasonable travel expenses incurred when attending for interview will be reimbursed. The
travel expenses for the successful candidate will be paid when they take up post and will
be included in their first monthly salary. Please note, however, reimbursement of
expenses shall NOT be made to individuals who refuse an offer of employment on the
grounds which, in the opinion of NHS Ayrshire and Arran, are inadequate. Travel
expenses will normally be reimbursed from the point of entry to the UK.
9.
REMOVAL EXPENSES
Removal expenses, associated with necessary relocation to the area, may be payable to
the successful candidate. It will be confirmed at interview whether or not removal
expenses are applicable. A maximum of £12,000 may be reimbursed for reasonable
removal expenses.

ANNUAL LEAVE
The annual leave entitlement appropriate to this grade is 6 weeks. Public holidays, as
determined for NHS Ayrshire and Arran staff, are granted in addition to the annual leave
entitlement.

SMOKING, ALCOHOL, DRUGS AND HEALTH AT WORK
NHS Ayrshire and Arran operates designated policies on *smoking, alcohol and drugs. All
staff are required to observe these policies.

Consultant Staff have an entitlement to a maximum of 30 days (including off-duty days
