ATHLETIC UNION CLUB INSTRUCTORS/COACHES APPLICATION FORM C2 In most cases Athletic Union clubs will need to have qualified competent coaches in order to run their sessions, and these coaches will need to be registered before any sessions are allowed to go ahead. To register instructors/coaches the following application must be completed, signed by two club officials and submitted to the Student Activities Manager (Students’ Union) along with all of the supporting paperwork. The following conditions apply: 1. All Instructors/Coaches must satisfactorily complete this application form and have it approved before they coach any sessions. 2. Provide all the required supporting paperwork and certificates. 3. All coaches must follow the attached responsibilities list at all times during Clubs training sessions or competition etc. 4. There must be a benefit to the Club. 5. All applications from the Club should normally be submitted together and before the first session of the academic year. 6. Applications received after the start of the academic year may be subject to a delay of up to 10 working days. Date: ……..…..…..… FORM C2 SAFETY RESPONSIBILITIES OF CLUB COACHES/INSTRUCTORS The guidelines below should be followed by all Club coaches and instructors at all Athletic Union Club sessions: 1. All coaches/instructors must be familiar with the contents of the Club Safety Policy and Risk Assessment. These documents must be adhered to at all times by coaches/instructors and all club members. 2. All coaches/instructors must be aware of the emergency arrangements for all Club sessions (including matches home and away) and the facility they are using. 3. All coaches/instructors are responsible for checking the condition and suitability of the equipment being used for the Club session. 4. All coaches/instructors should be familiar with the relevant and up to date guidance from their national governing body and ensure these guidelines are adhered to at all times (e.g. coach/instructor to participation ratio, ability levels considered etc). 5. All coaches/instructors should report any safety concerns to Mark Bennett (activitiesmanager.union@ncl.ac.uk) as soon as practicable after they are identified.
