For the safety and security of Northern Lebanon High School’s students and guests, we require that a student
bringing a guest the Northern Lebanon sponsored event from another school complete this form and present it at the
time of ticket purchase. Your guest must be 20 years old or younger. All students are required to present their
student ID upon entering the dance.

Once guest or NLHS student leave the activity, they will NOT be permitted to return.
