Research, Teaching, and Clinical Contributions A. Narrative description of research, teaching and clinical responsibilities Research interests include both clinical and basic laboratory efforts. Clinical research efforts are primarily targeted at examining outcome measures for patients undergoing procedures for cerebrovascular disease. This includes patients for carotid endarterectomy, brain arteriovenous malformations, and intracranial aneurysms. Development of combined modality management of endovascular and surgical techniques for the management of cerebrovascular disease is also a major research interest. Techniques have been developed to help minimize ischemic injury during neurovascular procedures. Studies continue in the use of mild hypothermia during surgical procedures. In the laboratory, efforts have focused on techniques and strategies to minimize the impact of brain injury or ischemia and are now being extended to fever control in the ICU as well as use during stroke. Specifically, strategies of hypothermia, induced systemic hypertension, and metabolic suppression using Ogilvy-4 a variety of techniques and agents are used. An in vitro model of retinal ischemia is used to screen techniques, and an in vivo model inducing middle cerebral artery stroke is used in rats and rabbits to test the different strategies and assess their efficacy. Report of Clinical Activities 1. Description of clinical practice My major field of interest is neurovascular disease. This is in the setting of a teaching hospital. Every patient is managed in conjunction with resident staff. The majority of patients harbor intracranial aneurysms, intracranial arteriovenous malformations, or extracranial and intracranial atherosclerotic disease. 2. Patient load Between 300 and 350 major procedures per year are performed. The majority of these patients harbor complicated intracranial vascular abnormalities, typically intracranial arteriovenous malformations or aneurysms, and are managed in conjunction with the endovascular and stroke services at the Massachusetts General Hospital. A total of approximately 500 patients are managed each year by the combined services. 3. Clinical contributions Constant efforts are made to review a well-kept data base to evaluate methods of clinical care and to modify techniques of clinical care for patients in order to maximize the outcome of a particular cerebrovascular problem while minimizing hospital utilization. In the majority of cases, patients are reviewed prior to treatment by a team of physicians made up of neurosurgeons, interventional neuroradiologists, radiologists, radiation therapists, and neurologists. This is done in an effort to provide patients with every available technique of treatment in an up-to-date fashion. Risks and benefits of each procedure are reviewed at the conference, and a final treatment strategy is agreed upon by the team members. This is then communicated to the patients, and treatment is instituted if appropriate. Patients are referred from local institutions as well as nationally and internationally.
