JOB DESCRIPTION Job Title: Service Improvement MDT Co-ordinator Grade: Accountable to: Trust Lead Cancer Manager/Lead Cancer Clinician Reports to: MDT Lead Cancer Clinicians Location: Hours: 37 per week Job Summary Increasing capability and capacity for service improvement across the MDT’s. To help facilitate service improvement with MDT’s who have identified areas where they can improve the experience and outcome of care for the cancer patient. Work with the MDT’s to develop their team around the patient journey and ensuring the patient and carer experience of care is central to every stage of their journey. Ensuring the MDT meetings run effectively and are well structured and all patients are treated and discussed according to evidencebased protocols. Principle Responsibilities Facilitate and co-ordinate the functions of the multidisciplinary team meetings. Ensure there is clear definition of roles and responsibilities for MDT members. Ensure the appropriate proportions of patients are discussed at MDT’s. Establish and ensure the continuation of effective communication. Help with the introduction and changes to pro-formas used to ensure all patients are discussed, treated appropriately and outcomes are recorded and reviewed. Ensuring patients’ diagnoses, investigations, and management and treatment plans are completed and added to the patient’s notes. Managing systems that inform GP’s of patient’s diagnosis, decisions made at outpatient appointment etc. Working with staff to ensure all patients have a booked 1 st appointment, investigation and procedure and record details of patients coming via a different route. Working with key MDT members to identify areas where targets are not achieved, undertake process mapping to identify bottlenecks. Identify PDSA cycle. Undertake demand and capacity studies where appropriate. Report changes to MDT’s on a monthly basis. Data collection and recording of data. Training with service improvement methodology for MDT Co-ordinators and other MDT members. Establish effective communication networks with clinicians and other members of the multidisciplinary team and actively participate in development and change to any systems. To manage the systems according to guidelines, monitoring milestones and submitting the required reports in the given format and required times. Keep comprehensive diary of all team meetings. Type reports of long term plans as advised by Clinical Leads. Prepare and type reports on behalf of the Clinical Leads/Clinical Directors/Directorate Managers for forthcoming Cancer Peer Review Visits. To report regularly to the lead clinician/trust on compliance/non compliance of standards. Record attendance at meetings Take minutes at the Cancer Steering Group and circulate papers to all members. Take minutes at the multidisciplinary meetings, type notes back in the required format and distribute to all concerned. The postholder will be expected to be instrumental in the development of databases to capture patient information and report this to the Clinicians on a weekly basis. Negotiate with all involved in cancer issues to establish good working relationships and improve the co-ordination of the patient journey. Keep typed notes of all issues discussed at the multidisciplinary team meetings. Inform Lead Cancer Manager of waiting times for patients when these exceed appropriate targets. Ensure lists of patients to be discussed at meetings are prepared and distributes in advance. Ensure all correspondence, notes, x-rays; results etc are available for the meetings. Ensure action plans for patient care are produced with agreed reviews. Assist in capturing cancer data on all patients and assist in the development of systems to complement the cancer audit system. Ensure members or their deputy are advised of meetings and any changes of date, venue, etc. Comprehensive knowledge of office procedures to assist in a coordinated approach to patient care. Ability to communicate with all levels of staff including senior clinicians. Work collaboratively with medical secretaries within the trust to ensure appropriate management of patient information and co-ordination of patient pathways, Distribution of memorandums, letters, minutes, patient details to all members of the multidisciplinary team. Effective management of time and resources. Work with members of the MDT’s to help reduce waiting times. Assist in developing systems to ensure compliance with the 2-week wait standard. To be proactive in meeting the demands of the service. To liaise with the Rapid Access Office regarding information of referral of patients via the 2-week cancer wait standard. To provide cross-functional cover for all multidisciplinary team meetings. To ensure that there is a co-ordinated approach to service delivery on both sites to promote good working relations. Quality Issues To assist in the development of care pathways. To ensure that directorate administrative and clerical functions are carried out to the highest quality standards by monitoring and improving services. To provide monthly reports to the Directorate for clinical governance purposes. To provide monthly reports to the Lead Cancer Clinician. To monitor patients progress through the systems. To advise Lead Cancer Manager/Clinical when milestone targets in the NHS Cancer Plan will not be met.
