Medical/Legal Urine Drug Collection Employment or Pre-employment Collection 1. Use a urine collection kit with dual plastic bottles and a Commercial Client Drug Screen Consent form (lab 355 05/03 NS). Contact the laboratory client service numbers to obtain kits and forms. 2. Prior to collection, complete the top portion of the Commercial Client Drug Screen Consent form. 3. The following areas must be completed: Employer Name and Address Name and phone number of a contact person at the company Client name Client date of birth Client gender Medical Review Officer name and address Reason for the collection Collection time Collection date Check a photo identification of the patient/client. Mark the appropriate box at the Photo Identification Checked question on the Commercial Client Drug Screen Consent form. Drug test panel required. 4. Have the client print his/her name on the Client Consent and Certification portion of the Commercial Client Drug Screen Consent form where it starts, “I _________, consent and agree……”. 5. Instruct the client to read the statement. Have them fill out the area for medications used recently. 6. Have the client sign and initial at the signature and initial area. Note: If confidentiality is desired for the Client, instruct the client to initial only. Initials and identification must match the specimen container. 7. Direct the client to leave all packages, purses, coats, jackets, bags, drink containers, or other objects that could conceal bottles or containers of urine or other chemicals outside of the rest room. 8. Instruct the client to collect the urine in the cup with the temperature sensitive strip, hand the completed urine collection directly to you before washing their hands or flushing the toilet. Once the specimen has been handed off, they may wash their hands and flush the toilet. 9. Check the specimen for volume of urine. At least 45 mls of urine must be collected. 10. If less then 45 mls of urine have been collected, proceed as follows. Have the client drink a cup of water (about 8oz). Recollect the specimen once the client needs to urinate, using a second specimen container with temperature strip. Follow the same instructions in steps 7 – 9. Label both the insufficient volume specimen and second collected specimen. DO NOT combine the specimens. Leave them in the separate collection containers. Submit both collections for analysis. 11. After checking the volume, check the temperature within 4 minutes of the specimen being handed to you. Record the Fahrenheit temperature on the Commercial Client Drug Screen Consent Form from the temperature strip on the side of the collection cup. The temperature must be between 90.5 F and 99.8 F. 12. If the temperature is not within this range, have the client recollect the specimen after drinking water. Submit both specimens. 13. Note the color of the urine on the form by circling the appropriate color. If the urine is colorless, have the client drink water and then recollect the specimen. Submit both specimens. 14. Observe the urine for the presence of sediment or precipitate. Circle the appropriate response. If unusual sediments or objects in the urine are present, have the client recollect after drinking water. Submit both specimens. 15. Divide the urine sample between the two plastic bottles by pouring from the collection cup into the bottles. Use 30 mL in one bottle and 15 mL in the other. In the presence of the client, cap the container, tighten the cap and place a piece of evidence tape across the cap of each container so that the lid cannot be removed without breaking the tape. 16. If a single sample was collected, the collector should check the Single Collection box and the client must initial in the space provided signifying that they understand that without a second sample they give up the right to have repeat testing done on an unopened container. 17. Label each container with the subject’s name and date of birth. If the Commercial Client Drug Screen Consent Form was signed using initials or identification code, use the initials or identification code instead of the name on the label. 18. Complete the test of the Commercial Client Drug Screen Consent Form. Note anything strange or special about the specimen by either putting “no” if nothing unusual or describe what is unusual in the space provided. 19. Record the location where the sample was collected. 20. Sign your name under the collection supervisor signature. 21. Provide the client with the client copy (refer to the distribution directions at the bottom) of the chain of custody form. 22. Place the sealed urine container(s) in a transport bag, seal the bag, and put the Toxicology Lab copy (refer to the distribution directions at the bottom) of the chain of custody and any other paper work in the pocket of the sealed bag. 23. A third copy is provided for the collection site. Retain this information for your records. 24. Place the bag in a lock box or other secured and monitored location. 25. If the test cannot be performed or transported within 24 hours, refrigerate or freeze the specimens and transport as soon as possible. 26. Deliver specimens to Toxicology.
