APPLICATION FORM

Return this form to the Mathematics Office (HHH 508) when complete.
1. Must have a declared mathematics major or minor.
2. Must be enrolled in UWEC for at least 8 semester credits including one math course.
3. Must be Freshman or Sophomore standing and have completed Mathematics 114 OR a
freshman that has completed a mathematics course numbered 215 or above (not Math 245).
4. Must have a cumulative G.P.A. of 3.25 or higher.
