Dear Valued UW BSN Alumnus:
We are pleased to share with you that the UW School of Nursing has been rated as the number 1 nursing
program in the nation since 1984. We couldn't have done it without you. Now, we need your valuable
input to evaluate and ensure the quality of the UW BSN program. Please take a few minutes of your time
to share your thoughts with us and respond to this questionnaire by June 3. Your time and effort in
completing and returning the BSN Alumnus Follow-up Questionnaire will be greatly appreciated. You may

We also request that you have one of your colleagues complete a brief questionnaire about your work
performance. This person should be someone in a supervisory capacity to whom you report and who
knows the quality of your work. Please complete the information at the top of the enclosed Colleague
Evaluation Questionnaire and then give the form and a business return envelop to a supervisory

All information in this questionnaire is confidential. All information received will be analyzed and
aggregated by group rather than individually. That is, the responses from each individual will only appear
