Denison Genealogy (1963) and/or the (1977) supplement may provide your complete line, and if so, please place the
assigned genealogy number opposite the respective ancestor. Once you have an ancestor with the assigned
genealogy number, you need not further record your ancestry for the purpose of this application.
If you are unable to trace your line completely, fill in this form as far back as you can and return it to the Society.
If we cannot readily provide the missing information from our records, we will be glad to arrange with you for a

Classes of Membership and Dues –
- Active Membership (includes adult descendent and/or spouse) - $25.00/year
- Active Family Membership (includes adult descendent, spouse, children or grandchildren under the age of 18)
- $35.00/year
- Active Junior Membership (includes descendent under the age of 18) - $15.00/year
Please send your non-refundable application fee, equal to one year’s dues with this completed application. This
