Pharmacy Services Contract
Between (your facility) and ________________________, RPH
Services will be provided as follows:
__________ visits to the facility by a registered pharmacist, to assure compliance with accreditation
standards and policies established by the pharmacy and therapeutics committee for medication
procurement, dispensing and administration.
Responsible for assuring compliance with all local, state, federal pharmacy laws.
Responsible for making sure pharmacy reference library is in compliance with credentialing agents, i.e.,
local, state, federal pharmacy laws.
Supervision of inventory controls for narcotics and all other drugs.
In-service training programs as requested.
Attendance at and participation in meeting of pharmacy and therapeutics committee and quality
improvement.
Establishment of the formulary and maintenance of same.
Routine monitoring of inventories for adequacy of supply, outdated medications and proper storage.
Replenishment of stocks of formulary drugs.
Recommendations of formulary drugs.
Continued assistance in purchasing and maintaining lowest possible acquisition cost.
Responsible for approval of all pharmacy policy and procedures.
Responsible for having back-up RPh. Provide name and telephone number.
Responsible for keeping facility notified of recalled drugs and devices.
Terms: This contract shall be for the period of twelve (12) months at a rate of $__________/year.
This contract may be terminated by either party with thirty (30) days written notice.

Job Description
Job Summary:
Supervise and coordinate all activities of client in accordance with client policies, Medicare standards,
state and federal laws and the standards of the profession. Establish purchasing policies and inventory
controls. Establish and maintain compliance with said established policies.
Performance Requirements:
Level of Formal Education Requirement: BS in pharmacy from an accredited school of pharmacy.
Completion of an accredited residency in hospital pharmacy is preferred. Three to five years as a
pharmacist preferred, preferably in a supervisory position.
License/Certification Required: Pharmacist License.
Personal Skills Necessary: Ability to manage a group of people in pharmacy setting; ability to listen and
interact with all surgery center personnel; ability to react calmly to stressful situation with various levels
of medical professionals; ability to communicate verbally.
Job Relationship: The pharmaceutical service shall be directed by pharmacist who shall have a qualified
back-up RPh, with 24 hour accessibility, and ancillary personnel who are competent in their duties and
sufficient in number to meet the pharmaceutical needs of client.
Responsibilities: The pharmacist is responsible for the maintenance of the quality, efficiency, and potency
of drugs administered to the patients of client. Sufficient controls shall be incorporated to minimize
errors while perpetuating a compatible working relationship between client personnel and the pharmacist
consultant.
Consultant Pharmacist: The consultant pharmacist will monitor client for providing quality standards in
the conduct, practice activities, and operation of its drug distribution system to ensure the public health,
safety and welfare.
