Job Purpose:

To deliver a high standard of nursing care and be
responsible for an allocated assignment

To prevent patients having unnecessary hospital admission,
enabling them to stay at home where possible.

To be responsible, where appropriate for the supervision,
deployment and teaching of junior staff/students.

To work within the policies of Bridges Healthcare and the
NMC code of conduct towards improving the health of
individuals.

To work as a member of a multi-disciplinary primary heath
care team the practitioner uses a combination of skills and
knowledge to identify health and nursing needs, and develop
strategies to promote health, whilst recognising cultural
beliefs and values of the individual.

To identify and respond to identified health needs gaps in
service provision, practice/community population.
Responsibilities
Clinical / Professional

To work as part of the Bridges Healthcare Team, to share knowledge and skills and
be responsible for the planning, implementation and evaluation of care, and
prioritise actions appropriately.

To assess care needs and develop of programmes of holistic care, systematically
reviewing effectiveness of care given.

To be responsible for a defined geographical population as assigned.

To ensure cost efficiency and effectiveness in all resources.

To be responsible for the delegation, allocation and supervision of patient care and
care undertaken by other team members.

To have basic I.T. skills.

To maintain confidential patient records and follow policies with regard to patients’
confidentiality.

To be responsible for communicating effectively and clearly both verbally and in
writing.

To participate in and facilitate clinical supervision.

To undertake appropriate developmental and health screening, offering advice as
necessary, and prioritise health-enhancing activities through targeting high risk
groups.

To set targets with the team for professional practice; evaluate the effectiveness of
outcomes; and define, maintain and review standards of care.

To be responsible for own professional knowledge and updating. Attend study
days, training sessions and refresher mandatory courses in accordance with PREP
requirements, and maintain a professional profile.

To maintain current registration of appropriate qualifications with the NMC and
maintain a portfolio, which demonstrates personal professional development.

To provide information to colleagues, professional groups and other trust
departments to achieve effective communication and ensure that all concerned are
informed of clinical practice issues.

To ensure accurate collection of data.

To manage body fluids to agreed protocols and policies.

To move and handle clients in accordance with training policies and protocols.

To adhere to Personal Safety Policy with regards to lone working, see Staff
Manual.

To work flexibly to meet the needs of individuals and carers in the community.

To raise or deal with any encounters in unacceptable practice through the
accepted channels.

To work closely and liaise effectively with other members of the primary healthcare
team, including GPs, nurses, hospital doctors, Social Services, Specialist Nurses
and other professionals to ensure that the health needs of the individual are met.

To negotiate appropriate cover by mutual agreement with colleagues.

To carry out administrative duties as required by the assignment and Bridges
Healthcare.

To perform risk assessment for individual care, environment, moving and handling,
fire training and personal injury.

To act as an autonomous practitioner with specialist expertise, implementing
complex care.
GENERAL
This post is one of continual development. The job description is intended as a guide to
the principle duties and responsibilities of the post and complements individual
objectives set in line with the agencies business objectives. Responsibilities will be
reviewed periodically in line with service priorities and duties may change or new duties
be introduced after consultation with the post holder.
