The 208 Consistency (if applicable) will be submitted with the Sanitary Facilities Application for Subdivisions – this submittal usually will take up
to 6 months for ADEQ approval – email the completed 208 Consistency to Edwina Vogan Vogan.Edwina@azdeq.gov

ADEQ Application for Sanitary Facilities to be filled out and signed by Client. Most of the information is acquired from
NOI, Final Plat, Engineer’s Design Report. (submittal coordinator generates) (Exhibit A)

Request an ADEQ Final Plat from Tom Christopher or Korey Hill signed & stamped by RLS & EOR (submittal
coordinator requests this final plat from survey when you have all other documents in hand)

Engineer’s Design Report (from project manager)
4.
208 Consistency Review – only for septic and onsite WW disposal – Edwina Vogan (602) 771-4606. (submittal
coordinator does this paperwork- OP-Doc#____)
5.
County conceptual approval (form 113) (if Septic, use Appendix B) (submittal coordinator generates and obtains
signature) (Exhibit B)
6.
Approval to Construct for Water (Once project is approved we receive the Approvals to Construct (ATC’s), The
water service provider generates this document) – Please see attached form from ADEQ (application)

Approval to Construct for Sewer (Once project is approved we receive the ATC’s, The water service provider

b. ADEQ form – Instructions for submittal of a Notice of Intent to discharge for an on-site wastewater treatment

Sewage Treatment Facility Capacity Assurance – original signed & dated. You type up form, using the Engineer’s
Design Report. (Exhibit F)

Capacity Assurance for Sewage Collection System – original signed & dated.

Geosoil Report if applicable for onsite WW disposal (from Project Manager)

APP – Completed for WW disposal greater than 24,000 gpd

ADEQ’s Water Service Agreement – original signed and dated (submittal coordinator generates and obtains

ADEQ’s Garbage service & Garbage Disposal agreement – original signed and dated (submittal coordinator
