Introduction
This 5PA post has become available following the flexible retirement of the present post
holder who will continue to work as the other partner of a proposed job share. The postholder will be jointly responsible for running a 23 bedded general adult psychiatry ward and
providing Medical Leadership to the Crisis and Home Treatment team serving. The London
Borough of Richmond. Lavender Ward is situated within the recently opened inpatient unit
at Queen Mary’s Hospital, Roehampton.
This post is part-time for 5 Programmed Activities (PAs) per week.

The Trust
Background
South West London and St George’s Mental Health Trust was formed in 1998. Springfield
Hospital in Tooting, the largest site from which the Trust operates, has provided mental
health services since 1841.
The Trust was originally known as the Pathfinder NHS Trust in 1994 and provided services
for Wandsworth and Merton. In subsequent years its strategy was to grow and to become
the main statutory provider of mental health services for South West London.
The Trust took on Putney and Roehampton services in 1998, Sutton Mental Health
Services from St Helier NHS Trust in 1999, and Kingston and Richmond services in 2001
as the former health authorities were disbanded and primary care trusts were established.
Kingston and Richmond services were in financial deficit at the time, and there were
serious concerns about the quality of older people’s services at Tolworth Hospital. The
different histories of services in different boroughs have been reflected in service models,
cultures and partnership arrangements.
Population
The Trust now serves a very diverse population of 1 million people in South West London,
with a range of BME communities and socio economic groups. In general the population of
SW London is more affluent than other sectors of London and many people in the boroughs
served by the Trust are highly educated and assertive in demanding the services they want.
We also know that there are also pockets of deprivation across south west London and
service provision is therefore carefully attuned to the requirements of the population as a
whole.

Richmond is an outer London borough with a population of 184,000. The River Thames
and the Royal Parks dominate the geography of the borough. The population trends
indicate a slight increase in the number of younger people in the borough with a projected
ethnic minority population of 7.4%. Although affluence is a key characteristic of the
borough compared with some other parts of the trust there are some pockets of social
deprivation. Health and Social Services have been integrated in Richmond for a number of
years and there is a healthy voluntary sector and service user movement, which works
closely with the trust. Richmond is a particularly attractive place to live, though property
prices tend to be high. This does present work force management challenges but not
withstanding this, staff turnover is generally low. There are excellent working relationships
within the consultant group.
St George’s, University of London
The Trust has a comprehensive training scheme for Core and Higher Specialist Trainees
and enjoys a close relationship with St George’s, University of London, an independent
School of the University of London.

SGUL, in association with St George’s Hospital, is one of five general combined
undergraduate/postgraduate teaching complexes within the Faculty of Medicine, University
of London. During the 1970s, long-term plans to relocate the Medical School and the
Teaching Hospital in Tooting were put into effect. The new Medical School opened in
October 1976 admitting pre-clinical students for the first time. The Medical course now has
an entry of 190 undergraduate students per year, plus 70 graduate students on the
Graduate Entry Programme. Since 1995, the range of courses offered has expanded to
include Biomedical Sciences and, in conjunction with Kingston University, Nursing,
Midwifery, Physiotherapy and Social Work. SGUL has a strong focus on research with
world class teams particularly in Infectious Diseases and Epidemiology. In 2006, a new
initiative, the South West London Academic Network, SWAN, was announced to further
research and teaching collaborations with Kingston University and Royal Holloway,
University of London.
Division of Mental Health
The Division of Mental Health comprises major research groups in community psychiatry,
eating disorders, forensic psychiatry and personality disorders, plus further groupings in
child and adolescent psychiatry, the psychiatry of addictive behaviour and disability. There
are further academic posts in the psychiatry of old age which are placed in the Division of
Developmental Sciences. In all, psychiatry represents a major specialty within the School.
3.1
The Post
The post will be managerially and professionally responsible to the Associate Medical
Director for Kingston and Richmond Borough
3.2
The role
The employer is the Trust whose main base is Springfield University Hospital. It is a part
time post. There are 5 Programmed Activities (PAs). 4 PAs for Direct Clinical Care (DCC)
and 1 PAs for Supporting Professional Activities (SPA). The clinical sessions are within
Lavender Ward and the Richmond Crisis and Home Treatment Team. The appointed
Consultant and job share partner will jointly provide Clinical Leadership to their part of the
service and both are accountable to the Associate Medical Director for:




Summary Description and Responsibility of the Post
Lavender ward
Lavender ward in a 23 bedded ward mixed sex ward with single sleeping areas and
containing single rooms within a complex the Queen Mary’s Hospital Roehampton; two
similar in patient wards serve catchment areas from within the Wandsworth sector of the
Trust.
Acute admission of patients of working age resident in the London Borough of Richmond
will be to Lavender Ward, with the exception of those under the care of the Assertive
Outreach Team (AOT). These patients will be admitted to the remaining Richmond In
patient beds in Orchids Ward at Tolworth Hospital.

There is close working between Lavender Ward and the Richmond Crisis and Home
Treatment Team including a regular clinical review meeting involving members of both
teams, with senior medical involvement.
The post holder will work closely with the CHTT and in patient team to improve the Clinical
Care and User Experience on Lavender Ward drawing on both knowledge and Evidence
Based Practice.
The joint consultant post will be supported by a full time Associate Specialist doctor and a
CT1-3 trainee from the Wey Valley rotation.
Cross cover on Lavender Ward is provided by trainees from within the LBR service and
also trainees working on the 2 Wandsworth inpatient wards.
Crisis and Home Treatment Team
Purpose of the Team
To deliver effective crisis resolution & home treatment in a range of settings and thereby
deliver a high quality alternative to inpatient care. In pursuit of delivering this service the
team will offer rapid response, 7 day initial assessment/screening which will provide a
gateway to the service. Individuals not admitted to the service would either be:

Discharged immediately following assessment (with signposting information to
other services and agencies where appropriate)

Care co-ordinated temporarily by the C&HT team, pending referral to another
borough team via the Borough Care Pathways planning meeting.
Referrals will routinely be taken from:

Other Richmond Borough teams (including CDAT), normally via Consultant to
Consultant or Team manager to Team manager level

General Practitioners

Approved Social Workers/Emergency Duty Team

A&E/Psychiatric Hospital liaison services

Mental health acute inpatient wards (Richmond patients only) for facilitated early
discharge
What the Service Does
Within Richmond borough the team will fulfil the following role:







Provide a rapid assessment service for all individuals requiring urgent same day
assessment
Deliver time limited interventions (normally 2-6 weeks) underpinned by clear treatment
objectives
Act as gatekeeper to acute inpatient beds with ALL new admissions coming through the
team
For individuals deemed appropriate for the service, provide an appropriate multi disciplinary
community based treatment 24 hours a day, 7 days a week.
Within the context of home treatment, provide a range of interventions, including
medication, psychological intervention, family interventions and practical help
Promote the health and well being of service users by providing education and information
on self-care, physical health, recognition of relapse indicators etc.
Participate fully in all acute inpatient treatment and planning processes to facilitate early
discharge and where appropriate extended leave arrangements.

Presents a monthly report to the Borough Care Pathways Planning Meeting
The period immediately following discharge from hospital or commencement of extended
leave is known to be a high-risk period for service users. The C&HT team will routinely
facilitate the early discharge of all inpatients unless there are clear reasons for this being
contraindicated, which have been highlighted by the discharge planning process.
Leadership and Staffing Arrangements
The Team Manager and Consultant working in very close partnership jointly lead the team. The
consultant will line manage all medical staff associated with the team and the team manager will
line manage all non-medical staff.
Service Commitments
 Clinical leadership of and full participation in the multi-disciplinary team providing mental
health services.
 Availability within normal working hours, five days a week for urgent assessments including
formal mental health act assessments.
 Involvement in any joint planning for Adult mental health services.
 Domiciliary visits as required.
 The appointed consultant will be jointly responsible for the clinical / educational
management of his / her junior medical staff. The Consultant is expected to maintain
Section 12 approval.
 The Consultant will provide and receive cover for and from consultant colleagues on leave
and absence.
 The postholder will be expected to have a full driving licence.
3.4
Job Plan
There are 5 Programmed Activities (PA’s). 4 PAs for Direct Clinical Care (DCC) and 1 PAs
for Supporting Professional Activities (SPA). The job plan will be reviewed annually within
the Trust Appraisal Process to reflect the needs of the service.

The post holder will be expected to be registered with the Royal College of Psychiatry in
respect of CPD and be part of the Adult Consultant Peer Group.
3.7
Administration
The Post holder will be expected to attend the joint Kingston and Richmond Consultants
committee and the joint medical staff committee which are held on a monthly basis at
Tolworth Hospital. There is a fulltime secretarial post attached to the CHTT and Lavender
Ward consultant and Team.
3.8
Clinical Governance
The Post holder will be expected to be compliant with the Trust Clinical Governance
requirements and participate in related initiatives where appropriate. This will include
clinical audit and review of outcomes to achieving National and local performance targets
and the post holder will be compliant with the Trusts risk management policy and
participate in the Consultant appraisal process.

Conditions of Service
The post will be offered under the terms and conditions of service for Consultants [England]
2003.
Salary: The Consultant salary scale commences on £74,504 p.a. pro rata rising through
pay thresholds to a maximum of £100,446 p.a. pro rata. Part-time Consultants are paid pro
rata, based on the number of agreed weekly Programmed Activities. The starting salary
will be determined in accordance with the Medical & Dental Whitley Council Terms and
Conditions for Consultants, Schedule 14. An on-call availability supplement will be payable
at the rate of Category A (3%) in reflection of the low frequency rota.
The Trust is entitled at any time to deduct from the post-holder’s salary, or any other
monies payable to the post-holder by the Trust, any overpayment of salary or wages,
under-deduction of charges, over-payment of holiday entitlement or any other sum which
the post-holder may owe to the Trust and, if this is insufficient, the Trust may require
payment of the balance.
Any offer of appointment to the post will be subject to the receipt of three references which
are satisfactory to the Trust, confirmation by the Occupational Health Department that the
pre-employment health screening is acceptable and immunisation against infectious
diseases as may be required.
