LEGAL IDENTIFICATION All students (new and returning) must have a copy of a Birth Certificate and Social Security Card in their file. STATE HEALTH LAW – ENROLLMENT All students (K4-12) (new and returning) in Pasco County Schools (public or private) MUST have a complete up-to-date certificate of immunization/physical requirement PRIOR to enrollment. PHYSICALS Any student participating in the Academy sports program must have a yearly physical exam report by the end of August. Students, returning and new, must have a physical exam report submitted by the end of August. A physical exam report (not an appointment or a promise to go), completed within one year prior to enrollment (first day of attendance in school) is required before enrollment, for any student who has never attended school in Florida in the past, including kindergarten and out of state transfers. Homeless students are the only exception to this rule. The immunization record must show that the student has met the minimum state requirement: Pre=kindergarten Varicella (chicken pox) vaccine or date of disease (year) as verified by parent or physician Hepatitis B (HBV) series Hib Up to date for age for DtaP, Pollo, and MMR Kindergarten, 1st, 2nd, 3rd, 4th, 5th and 6th grade students 5 doses DPT (diphtheria, pertussis, and tetanus) 4 doses polio vaccine (IPV, OPV) 2 doses MMR (measles, mumps, rubella) Hepatitis B series (HBV) = given over a 6 month period Kindergart4en, 1st, 2nd, 3rd, 4th and 5th grade students must also have the varicella (chicken pox) vaccine or date of disease (year) as verified by parent or physican 6th through 12th grade students 5 doses DPT (diphtheria, pertussis, and tetanus) 4 doses polio vaccine (IPV, OPV) 2 doses MMR (measles, mumps, rubella) Hepatitis B series (HBV) – given over a 6 month period 1 dose Td (tetanus, diphtheria) Immunization waivers Students who are enrolling, and who have previously attended school in Florida, are granted a 30 day period of time for their previous records to arrive. Schools will have them complete a “30 day waiver” form. Parent/guardian signs a statement that they agree that when the records from the other school arrive, if the information in the record is incomplete, they will get the needed immunnizations for their child. Note: *30 day waivers are not applicable for first time enrollees (ie…kindergarten or pre-kindergarten) or Grade 7. Homeless students are the only exception to this rule. Florida Statute gives homeless students a 30 day grace period to comply. We are once again asking that you take care of this important information. The Pasco County School District and the Pasco County Health Department require all students to have the proper immunizations, Birth Certificates, and Social Security numbers in their school’s student files. Please help us with keeping what Pasco County requires by bring to the office what we have asked for.
