Dear Parents, Guardians, and Caretakers:
Everyone at Golden Hill is looking forward to welcoming your child and you to Kindergarten and
the Golden Hill Elementary School community! It is our goal to make both you and your child comfortable
and excited about your kindergarten experience.
Enclosed please find a number of forms that need to be completed for the Kindergarten registration
process. These are being sent to you ahead of time so that you can complete them prior to coming for a
registration appointment. This will also give you the time to reflect on the information asked so that the
school can meet your individual child’s needs.
Kindergarten Registration is scheduled for Thursday, March 20, Friday, March 21,
Monday, March 24, and Tuesday, March 25, 2014. Appointments are scheduled every 15 minutes beginning
at 9:00 A.M. Please contact the school (651-4407) to schedule a registration appointment. Please try to
attend your registration appointment without your child, as we have found this seems to work well for all
involved. Mrs. Kathy Derevjanik, registrar, will be meeting with you and you will also have the opportunity
to meet with our school nurse, Mrs. Marlene Lysack, R.N., to review the blue health forms and any other
information you may wish to share about your child.
Save this date! Both your child and you are invited to attend a special afternoon orientation on
Friday, June 13, 2014, at 12:00 noon.
Kindergarten screenings are scheduled for April 29, 30 and June 1, 2014. At registration, we’ll
schedule a personal appointment with you. We’ll discuss the purpose and content of this educational
screening at registration.
The procedures and requirements for entrance into Kindergarten are listed below:
Your child must be five (5) years of age on or before December 1, 2014.
Verification of your child’s date of birth MUST be presented at the time of registration.
For example: Original Birth/Baptismal Certificate with a raised seal.
Proof of Residency. For example: tax bill, mortgage statement, utility bill, driver’s license, etc.
New York State law states that each student entering school MUST have the following
immunizations:
A. 3 or more doses of diphtheria toxoid containing vaccine (DTP, DTaP).
B. 3 or more doses of oral poliovirus vaccine (OPV) or 4 or more doses inactivated polio vaccine
(IPV).
C. 1 dose of mumps vaccine administered on or after 12 months of age.
D. 1 dose of rubella vaccine administered on or after 12 months of age.
E. 2 doses of measles vaccine, first dose administered on or after 12 months of age.
(C, D, and E preferably as MMR)
F. 3 doses of hepatitis B vaccine.
G. 1 dose varicella (chicken pox) vaccine or physician signed document stating child has
has chicken pox.
Immunization records MUST be presented at the time of registration. NO CHILD IS
ALLOWED TO ENTER SCHOOL WITHOUT PROPER IMMUNIZATIONS,
VERIFIED BY A DOCTOR.
